All pilot ladders used for pilot transfer shall be clearly identified with tags or other permanent marking so as to enable indentification of each appliance for the purposes of survey, inspection & record keeping. A record shall be kept on the ship as to the date the identified ladder is placed into service & any repairs affected. 74SOLAS(20)Con.V/23.2.4. PSCO discovered 01 pilot ladder with an unidentifiable placard. It was also determined there was no inspection certificate for said ladder.
Condition: Improper/Lack of Maintenance
Action required: 10 - Deficiency Rectified
Resolved 13 January 2022
Resolution: Vessel removed unsat pilot ladder from service and replaced with a spare that was onboard. Replacement ladder condition was satisfactory as supported by current Certificate of Inspection that corresponded with the identificaton tag on the ladder.
