Portable fire extinguishers which have been discharged shall be immediately recharged or replaced with an equivalent unit. PSCO discovered vessel using Yamato powder fire extinguishers, spare powder is manufactured by blok. Yamato portable fire extinguishers manual and instruction required only Yamato powder to be used. 74 SOLAS (20) II-2/14.2.1.2

Resolution: During examination on 23MAY2025 it was found that vessel no longer even used those tyoes of extinguishers and those that they carried had appropriate servicing certificates.

The safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the company. PSCO discovered crew did not notify company regarding the inoperable battery charger in the lifeboat. 74 SOLAS(20)IX/3.1 ISM Code 9.1

Resolution: During examination 23MAY2025 vessel showed that SMS had procedures for reporting issues up to company.
