Each rescue boat embarkation and launching arrangement must permit the rescue boat to be boarded and launched in the shortest possible time. The vessel's rescue boat launching davit experienced a failure of a hydraulic fitting, resulting in the discharge of hydraulic oil and loss of the ability to launch and recover the rescue boat. Repair to the satisfaction of certificate issuing authority.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure the ship in all respect will remain fit to proceed to sea without danger to the ship or persons on board. On 06 NOV 2024, vessel reported loss of starboard engine propulsion. Prior to departure, vessel will correct defect(s) discovered, perform appropriate testing, and demonstrate proper operation of all previously failed equipment to the satisfaction of the RO and submit report to the OCMI. The Coast Guard upon receiving the report will make a determination as to if a Marine Inspector will attend the vessel, prior to departure.

In the event of an accident to a boiler, unfired pressure vessel, or machinery tending to render the further use of the item unsafe until repairs are made, or if by ordinary wear such items become unsafe, a report shall be made, by the chief engineer immediately to the Officer in Charge, Marine Inspection, or if at sea immediately upon arrival at port.
Provide a causal-analysis report identifying the contributing factors that led to the failure of the starboard CPP, which can only go full ahead when engaging follow up, as well as a report from your recognized class society certifying that any necessary repairs have been satisfactorily completed and the vessel is fit for its intended route and service. Any movements in the Port of New York or New Jersey shall be conducted IAW a tug assist plan submitted and coordinated through VTS.
