Vessel is built and equipped with an INERGEN fixed gas fire-extinguishing system. During the annual tests of the system, critical failure in multiple discharge valves were identified. Vessel shall not operate or energize any internal combustion engines or engage in any hot-work or spark producing activity/maintenance on board the vessel. INERGEN test to be witnessed and finalized to the satisfaction of the attending USCG MI. 74SOLAS(14) II-2 10.5.2.1

(a) Before a vessel may be used as a nautical school ship, it shall be inspected by the Coast Guard to determine that the hull, boilers, machinery, equipment and appliances comply with the regulations in this part. (b) Every nautical school ship subject to the regulations in this part shall be inspected annually, or oftener if necessary, by the Coast Guard to determine that the hull, boilers, machinery, equipment and appliances comply with the regulations in this part. The vessel's COI has lapsed without renewal. COI-Renewal Exam to be conducted and full COI issued prior to operations. 46CFR 167.15-1

One exception: The Emergency Diesel Generator may stay in Auto for use in an emergency or loss of main bus. INERGEN test to be witnessed and finalized to the satisfaction of the attending USCG MI.

Enclosed space entry and rescue drills should be planned and conducted in a safe manner, as appropriate, the guidance provided in the recommendations developed by the Organization. Vessel failed to provide any gear or procedures during drill. Vessel to provide IAW regulation to the attending USCG MI prior to the carriage of passengers. SOLAS III 19.3.6.1

Coast Guard Sector New York has received a report that the vessel is operating under reduced propulsion due to a faulty converter on one of their drive motors. In the interest of safety, the Captain of the Port New York is requesting a tow plan be submitted in writing to this office for review prior to transiting within the Port of New York and New Jersey. Until the drive motor converter is replaced the following conditions apply: 1.) All transits are made when visibility is two nautical miles or greater, and must be coordinated through the (VTS) Vessel Traffic Service. 2.) In addition, if at anchor, (1) the vessel must have tugs alongside during all tidal changes, (2) when winds sustain 25 knots or more (3) at other times as directed by VTS.

Ready availability of water supply: to the satisfaction of the administration. 2.1.4.2 A valve shall be fitted to serve each fire hydrant so that any fire hose may be removed while the fire pumps are in operation. The vessel has outfitted each fire station with an non-gated siamese connection. While the main plug valve exists, there is great safety concern in practicality. If individual teams are to use both hoses from one plug, there needs to be isolation to prevent incident. If one hose was to be lost by rupture, both teams would be compromised. Vessel to deploy alternate arrangements in the isolation of the hoses to remedy the unnecessary risk. To be completed to the satisfaction of the attending USCG MI. 74SOLAS(20) 2.1.2.2 74SOLAS(20) 2.1.4.2

Each state maritime academy shall provide two chapters of the NSMV SMS. Specifically chapters 2 and 3 are provided by each school. Manning conditions, including in-port berthing of cadets, dockside operations, and other specific education operations are listed in the State Maritime Academy (SMA) Safety Management System (SMS) and approved by the local Officer in Charge of Marine Inspection. Provisional approval provided pending revisions provided on this date.

Notwithstanding the requirements set out elsewhere in the Convention, the full functionality of the safety systems listed below shall be available from the safety centre ...fire detection and alarm systems. The fire alarm panel at the safety centre (bridge) shows numerous faults. Provide proof of non-fault operation. Extended due date till 1 March 2025. SOLAS II-2 23.6.10

A fixed fire detection and fire alarm system shall be so installed in and arranged as to provide smoke detection in service spaces... The Category 11 Moderate Fire Risk Space Walk-In freezers did not have smoke detection.

Each fast rescue boat shall be served by a suitable launching appliance complying with section 6.1.7 of the Code. The DBA refers to the SPS Code which refers to SOLAS III passenger for vessel. Reg 26 is additional requirements for ro-ro passenger vessels which is defined as a vessel with a special category space.

Only corridors, lifts, public toilets, special category spaces,... are authorized to have direct access to these stairway enclosures. The aft stair tower has access from two small lounges and an elevator mechanical access.

The width, number, and continuity of escape routes shall be in accordance with the requirements in the Fire Safety Systems Code. Doors in escape routes were found to be less than 900 mm in width.

Escape doors from public spaces that are normally latched shall be fitted with a means of quick release. Doors were fond in public spaces thatdid not have a "panic bar".

A set of Coast Guard approved automation testing procedures shall be maintained on board and made available on request. PSTPs are pending shipboard testing and have not received final approval.

General alarm bells. General alarm bells shall be marked in not less than 1/2-inch red letters: “General Alarm—When Bell Rings Go to Your Station.” General Alarm Equivalent PA/GA did not have markings.

Where... difficulty is experienced determining... the minimum fire integrity values, such values shall be determined to the satisfaction of the administration. The aft mooring deck space is classified as Category 5 but has been determined to be Category 11.

The minimum fire integrity of... all decks shall be as prescribed in tabels 9.1 and 9.2. The Mapei deck covering was not approved as installed.

Video and audio surveillance equipment... ensuring the visibility of every door in each such passageway. Vessel does not have audio capability and every door is not visible. Prior to 24 December 2024.
