A safety system shall be provided to ensure that serious malfunction in machinery or boiler operations, which presents and immediate danger , shall initiate an automatic shutdown of the plant and that an alarm shall be given. During the fuel leak-off test on the #3 MDE the fuel oil sensor failed to activate during the test in the seated position. Vessel to repair the fuel oil sensor and test to the satisfaction of the OEM (Cummins servicing technician) and submit the technician's report attesting to proper repairs/operation.

During the 6-month interval testing the pressure test of the cylinders revealed (02) inergen bottles that revealed a severe pressure drop due to leak-off. Servicing company to provide service report attesting to the total number of cylinders failed; attest to renewal of the cylinders that failed during pressure testing and a causal analysis of the cylinder heads attached to the failed cylinders along with a summary of findings from the manufacturer and recommended preventative measures.

The Deck Officer’s offices are considered to be a “room-in-room” arrangement and are not provided with direct access to an acceptable means of escape. An equivalent level of safety should be provided for escape purposes. Observed both flange connection breaches allowing water to free-flow from the faulty gaskets in both locations compromising the systems pressure and creating a safety hazard impacting personnel and means of egress to port side lifeboats. The following requirements shall be met: 1.) Identify the cause of the breach in both flange connections on the fire main. Make repairs to original. 2.) During annual testing of the Fixed Firefighting System (Inergen) the Inergen space/control space filled with Inergen, causing the room to become oxygen deficient (15-19.5%) thus creating a dangerous situation for personnel. Provide root cause analysis of this issue. Additionally the system has been rebuilt due to debris in lines and miscellaneous parts, the vessel must prove the system is back together as per the approved plans and conduct approved pressure testing on effected system and piping in the presence of a Coast Guard Marine Inspector. No further testing will be conducted until all parties are satisfied with results of the RCA and proper system verification has been conducted as described above.

No fire detection is provided in the Reefer Provisions Storage 1 and 2 on the 2nd Deck. SOLAS II-2/7.5.2 requires fire detection and alarm system in service spaces, control stations and accommodation spaces. Per the fire risk categorization labels under SOLAS II-2/9.2.2.3.2, refrigerated chambers fall under category (11) ("auxiliary machinery spaces, cargo spaces, cargo and other oil tanks and other similar spaces of moderate fire risk"). As these two spaces were correctly categorized as category (11), rather than category (13) ("store-rooms, workshops, pantries, etc."), SOLAS II-2/9.2.2.3.2 was not applied. However, USCG MSC PRG SOLAS-37 requires fire detection and suppression to be installed within category (11) refrigerated storage spaces.

During plan review of Patriot State, it was found that the aft mooring deck (SFP Plan NSMV-635-B801, Rev.6, Sheet 23 of 34) is shown as a Category 5 open deck space. For this categorization to be correct it must be at least 30% open to the outside of the ship and the overhang must be no more than 10m.Based on the 25% opening, the aft mooring deck is an enclosed space and is to be protected by a manual or automatic deluge system. Provide a plan to come into compliance to the satisfaction of the local OCMI. Prior to sailing, provide a satisfactory risk mitigation plan subject to approval of the cognizant OCMI.

Hydrants shall be so placed that the fire hoses may be easily coupled to them. It was found that fire stations #13 #66 appear to be installed in a position that could cause the hoses to kink, in addition some of the stations are not hooked up, in interior locations fire hoses shall be always connected to the hydrants. Demonstrate to the attending USCG that the hoses can be easily charged with out kinking and can easily be changed out if needed.
