Issued 1 May 2021 Resolved
No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Prior to loading cargo, submit a plan for permanent repairs from the collision that occurred on 01MAY2021. Vessel is authorized to transit IAW transit plan provided to Houston, TX in order to offload cargo and back to FMT shipyard in New Orleans to conduct repairs. If there is any deviation from transit plan, notify cognizant OCMI immediately.

Resolved 24 May 2021
Resolution: Conducted a visual exam of all repairs and witnessed air test at 1.0 PSI. Test satisfactory.
