Vessel was mobilized for a backload operation with gasbuster that had Class 1 Div 1 zones over most of deck including where lights, electrical switches, engine room ventilation, escape hatch, etc were inside HAZ ZONE. 705 - Prior to backload operations.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. Vessel was observed with aft leg removed completing weld repairs to leg tower and leg pipe internal support structures. No notification to cognizant OCMI was made. Marine inspector to verify repairs and non destructive tests are complete before vessel departs from shipyard.

The following piping systems must be hydrostatically leak tested in the presence of a marine inspector at a pressure of 1.5 times the maximum allowable working pressure of the system: Any Class I, I-L, II-L piping. Several jacking system hydraulic brake pressure and brake lines were being replaced.

Note that there is an automatic bilge pump installed in this area used to accumulate water and pump directly overboard. Oil was observed discharging directly overboard from the overboard piping system. 46 CFR 176.25-10(7)(b).

The MI observed significant deck plating and support station cracking in vicinity of the aft port leg brace. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. The COI anniversary date is September 12 and expires on September 12, 2027. The vessel was required to have an annual inspection no later than December 2024. The vessel failed to schedule an annual inspection until February 18, 2025, and operated for approximately 2 months without a valid COI. Vessel shall ensure that it does not operate without a valid COI.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected… The port-side shaft seal compartment contained excessive oily water. This compartment is equipped with a portable bilge pump connected to piping leading directly overboard. Since the shaft seal allows water to accumulate in the compartment and requires periodic operation to drain, the vessel must ensure no oil accumulates in the space to prevent discharging oil overboard.
