Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Company has 5 vessels with expired CODs. All applications have been submitted. Provide compies of CODs once delivered for vessels HUNA KAAWU, GATHEENI, KEET, POINT SOPHIA, and PORT FREDERICK. 46 CFR 182.520

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. Starboard aft fuel shut off valves installed in manner which contacts starboard forward valve preventing full closure. Adjust starboard aft fuel shut off valve so valve can be closed without interference. 46 CFR 182.455 (b)(4)

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter. Batteries used are not per manufacturer. Restrict vessel to daylight operations until appropriate batteries are installed. 46CFR180.70(d)(1)

Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy. Float light missing corrosion resistant clip.

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. The port side, forward cabin heater exhaust pipe is missing a section of exhaust lagging. Repair or replace missing exhaust lagging. 46 CFR 177.970

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. (1) Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. (2) Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. (3) Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy. Vessel lifering buoy is missing the float light. Provide CG Approved float light, with lanyard and corrosion resistant clip. 46 CFR 180.70

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Vessels alco test strips are expired, renew expired test strips. 46 CFR 185.212

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB hydrostatic release has expired. Renew hydrostatic release. 46 CFR 180.64

Public address systems. (a) Except as noted in paragraphs (d) and (e) below, each vessel must be equipped with a public address system. (b) On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. (c) A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. (d) On a vessel of not more than 19.8 meters (65 feet) in length, a battery powered bullhorn may serve as the public address system if audible throughout the accommodation spaces of the vessel during normal operating conditions. The bullhorn's batteries are to be continually maintained at a fully charged level by use of a battery charger or other means acceptable to the cognizant OCMI. (e) On a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, a public address system is not required if a public announcement made from operating station without amplification can be heard throughout the accommodation spaces of the vessel during normal operating conditions, to the satisfaction of the cognizant [name withheld] stbd side, bow PA speaker is not working. Repair or replace faulty speaker. 46 CFR 184.610

Vessel is required to have onboard 100% IBA iaw D17 Policy 16714.1A. Vessels IBA's are not onboard the vessel awaiting return from the service facility. 46 CFR 180.25

Vessel is required to have portable fire extinguisher serviced annually by a state certified fire servicing company. Vessels portable fire extinguisher have not been serviced within the past 12 months. 46 CFR 176.810

Each person employed in a safety sensitive position must be pre-employment drug tested and enrolled in the companies random drug testing program. Operator could not provide proof of pre-employment or random testing for all marine employees. 46 CFR 16.210 & 16.230.

Vessel is required to have a visual indicator for the bilge high level alarm. Light at the operating station is inoperable. 46 CFR 182.530(a)

Vessel is required to have 03 fire buckets. Vessel is missing 01 fire bucket. 46 CFR 181.610

Vessel is required to have 01 ring life buoy light. Light is inoperable because the battery is missing. Must be fixed prior to night operations. 46 CFR 180.70(d)

Vessel is required to have 01 ring life buoy that is properly labeled. The ring life buoy was missing the vessel name. 46 CFR 185.604(b)

Vessel is required to have a sound producing device. Ship's horn is inoperable. 46 CFR 185.320

Vessel shall label all fuel shut-off valves. 03 diesel tanks for heating are missing labeling. 46 CFR 185.608

Vessel is required to have a logbook for lifesaving equipment maintenance, EPIRB testing and drills. Vessel logbook is missing. 46 CFR 185.702

Vessel shall have up-to-date tides and current tables. Vessel was missing the 2021 version. 46 CFR 184.420
