Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. (iv) The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Records of monthly portable fire extinguisher inspections were not provided.

Watertight hatches must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT HATCH—KEEP CLOSED” Water tight hatches are not labeled.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part. (d) Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Drill log provided does not provide a description of the drills, the date of the drills and not all crew members on board have participated in drills.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Maximum number of passengers allowed on upper deck not labeled.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Marking for life jacket number stored inside a container worn off.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Retroreflective tape on ring buoy worn.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. Aspirin expired in first aid kit

The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules (33 CFR part 84), so as to be visible at the following minimum ranges: (b) In a vessel of 12 meters or more in length but less than 50 meters in length: A masthead light, 5 miles; Vessel mast head light working intermittently.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Wing nut are being used to connect to battery terminals.

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Waterligth battery not working. Life buoy lanyard is frayed.

A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. MIs observed that after activation of the pre-engineered fixed gas fire extinguishing system the engines were capable of being restarted prior to activation of the reset device. Vessel must adjust system so that after automatic activation the engines can only be restarted after the reset device is used. 46 CFR 181.420(b)(3)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. MI observed expired contents within their first-aid kit. Vessel must remove and renew first-aid kit with non-expired contents. 46 CFR 184.710

Except as provided in 173.17, no person may use a vessel to which this part applies unless: (1) It has a number issued on a certificate of number by the issuing authority in the State of principal operation. Vessel is not State registered. Vessel must obtain State registration. 33 CFR 173.15(a)

An inflatable liferaft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, … . No later than the month and year on its servicing sticker affixed. PSCOs observed that the vessel’s inflatable liferaft is overdue for service. Liferaft requires servicing. 46 CFR 185.730(a)(1)

An adult life jacket must be provided for each person carried on board a vessel. PSCOs observed that the vessel is short one adult lifejacket. Vessel must obtain one additional lifejacket. 46 CFR 180.71(a)

A vessel must carry one floating waterlight, unless it is limited to daytime operation. PSCOs observed that the waterlight onboard the vessel did not operate. Vessel must obtain an operational waterlight. 46 CFR 180.70 (d)

Each floating waterlight … , on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. PSCO observed that vessel is missing the corrosion-resistant clip. Vessel must obtain a corrosion-resistant clip. 46 CFR 180.70(d)(3)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. PSCOs observed that the hydrostatic release unit used for the liferaft was never stamped upon installation, and no evidence could be produced by the vessel proving that it is not overdue for inspection. The EPIRB hydrostatic release was past due. Vessel must replace and mark each new disposable hydrostatic release unit. 46 CFR 185.740(b)

For fixed-gas fire extinguishing systems, … . The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. PSCOs observed that the fixed-gas fire extinguishing system has not been serviced as required. Vessel must have fixed-gas fire extinguishing system serviced. 46 CFR 176.810(b)(2)

Except on a vessel limited to day-time operations, the compass must be illuminated. PSCOs observed that the compass could not be illuminated. Vessel must add illumination to the compass. 46 CFR 184.402(c)

To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in 176.500(b)(1) [annual inspection within 3 months before or after each anniversary date] and your Certificate of Inspection must be endorsed. Vessel is outside of three month window for Inspection and is not permitted to operate until a COI Renewal inspection has been completed. 46CFR176.502

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel is prohibited from operations until a drydock examination and an internal structural exam has been completed by a US Coast Guard Marine Inspector. 46 CFR 176.600(c)(1)

A vessel must undergo an annual inspection within 3 months before or after each anniversary date. Last annual examination was conducted on 14NOV2020 and vessel is not permitted to operate until an inspection has been completed.

Contact cognizant OCMI to conduct a New to Zone Inspection prior to the carriage of passengers. Vsl was constructed in Sector New Orleans OCMI zone. Vsl will depart Sector New Orleans OCMI zone and will be home ported in Sector Miami OCMI zone, where the VSL will coordinate and conduct the New To Zone COI inspection. 46CFR176.840 701C
