The fire detection system shall be installed, tested, and maintained in accordance with the manufacturer's design manual. Vessel did not have an adequate means to test their fire detection system, which included lack of an allen key to reset the pull station in the engine room. Vessel operator needs to provide documentation and record in their TVR that they have completed a satisfactory test of their fire detection system.

A towing vessel greater than 26 feet must be fitted with a floating electric water light approved under series 161.010 or 161.110. The starboard floating electronic light was missing two screws on the housing. Vessel operator must replace the two missing screws to ensure that floating electronic light is able to maintain buoyancy when placed in the water.

The steering compartment contained hydraulic oily water mixture, which covered the vessel's steering mechanism. Worn areas on the exterior of the compartment have created entrance points for rainwater to enter the space. Vessel operator must drain the hydraulic oily water mixture and inspect/repair the deck plating on the exterior of the compartment to ensure weather tightness.

Vent pipes for integral fuel tanks that discharge to the weather decks must be fitted with a 30-by-30 mesh corrosion-resistant flame screen. Flame screens were missing on the generator day tank and waste oil tank ventilation piping on the weather deck. Vessel operator must fit ventilation piping with flame screens as described in 46CFR143.265(c).
