46 CFR 111.105-3 (b) Electrical installations in hazardous locations must comply with paragraph (b)(1), (2), or (3) of this section. (1) NFPA 70 Articles 500 through 504 (incorporated by reference, see § 110.10-1 of this subchapter). Equipment required to be identified for Class I locations must meet the provisions of Sections 500.7 and 500.8 of NFPA 70 and must be tested and listed by an independent laboratory to any of the following standards:(i) ANSI/UL 674, ANSI/UL 823, ANSI/UL 844, ANSI/UL 913, ANSI/UL 1203, ANSI/ISA 12.12.01, or ANSI/UL 2225 (all incorporated by reference, see § 110.10-1 of this subchapter). (ii) FM Approvals Class Number 3600 (1998), Class Number 3610, Class Number 3611, Class Number 3615, or Class Number 3620 (incorporated by reference, see § 110.10-1 of this subchapter) (iii) CSA C22.2 Nos. 0-10, 30-M1986, 157-92, or 213-16 (incorporated by reference, see § 110.10-1 of this subchapter). The conduit on the port lower hose reel was added or replaced at some point and the material does not meet the above sections.

Drills and exercises must test the proficiency of vessel personnel assigned security duties at all MARSEC levels and the effective implementation of the vessel security plan. They must enable the vessel security officer to identify any replated deficiencies that need to be addressed. No drills or exercises have been conducted in the past 3 years. The VSO must ensure that at least one security drill is conducted at least every 3 months. No drills have bene conducted in the past 3 years

Exercises Must be conducted at least once each calendar year, with no more than 18 months between exercises. No exercises have been conducted in the past 3 years.

Unless otherwise specified in the section, the VSO must keep records of the activities as set out in paragraph b of this section for at least 2 years and make them available to the CG upon request; For each annual audit, a letter certified by the CSO or VSO stating the date the audit was completed. There are no records or an audit ever being completed at the time of the inspection.

A vessel owner or operator required by 155.1040 (unmanned tank barges) to have a response plan shall conduct exercise as necessary to ensure the plan will function in an emergency. Both announced and unannounced exercise requirements must be included. The following are the minimum exercise requirements for the vessels covered by this subpart (1) qualified individual notification, (2)Emergency procedures exercise, (3) Shore based spill mgmt tabletop exercise, (4) OSRO equipment deployment exercise, (5)An exercise of the entire response plan, which must be conducted every 3 years. Tank barge has not conducted any of these drills or exercises in the past 3 years and could not produce records of the OSRO deployment exercise.

ANNUAL INSPECTION, Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel's anniversary date on its COI is April 7th. Vessel has exceeded 3 months after the anniversary date. Vessel is restricted from fueling/bunkering operations until a satisfactory annual/periodic inspection can be performed.

You must contact the cognizant OCMI to schedule an inspection at a time and place which he or she approves. Owner failed to contact the cognizant OCMI and make the Tank Barge available for inspection prior to the COI anniversary date (07JUL).

Install visual and audible bilge high level alarm at the operating station for the machinery space.

Attach approved retro-reflective tape and properly mount life rings so not secured to vessel.

Provide approved float light for life ring.
