Connection rod failed on starboard main diesel engine, resulted in a hole in the engine block and for the engine to catch fire. Starboard main diesel engine is inoperable. Fire damaged installed lighting and electrical wiring in overhead of engine room. Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions.

Port captain reported vessel electronic throttle did not respond when captain tried to stop vessel way resulting in the vessel collision with the companies’ hopper barge. Machinery and electronic systems must be designed and maintained to provide safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Verify and attest to the proper operation of the vessels throttle controls to the satisfaction of the TPO.

Resolution: Technician conducted troubleshooting procedures. Throttles were found to be operating as designed. Problem could not be replicated.

Owner reported vessel received damage to head log and second deck during a collision with a hopper barge during underway operations resulting in fractured welds and a 3 foot inset on bow. The owner or managing operator of a towing vessel that has selected the TSMS option must examine to ensure that the vessel and its equipment are suitable for the service for which the vessel is certificated. Conduct repairs to the satisfaction of the attending RO.

Resolution: Witnessed satisfactory repair inboard #1 tow Knee, involving insert of hesd log, slope sheet, walking deck and leading edge of Texas deck. All repairs pneumatically tested, visually inspected, all test satisfactory. GS
