Fixed fire-extinguishing systems shall be installed having due regard to the fire growth potential of the protected spaces; and fire extinguishing appliances shall be readily available. PSCO observed 18 of 89 pneumatic CO2 cylinder valves in the fixed firefighting system were observed to have safety pins installed, while the remaining 71 valves were free of safety pins. 74 SOLAS (20) II-2/10.1.1.1-2

Access opening in bulkheads at ends of enclosed superstructures shall be fitted with doors...weathertight when closed. The means for securing these doors weathertight shall consist of gaskets and clamping devices...the doors shall be arranged that they can be operated from both sides of the bulkhead. PSCO observed worn gasket, corrosion, and observed daylight from within ship. Additionally the door was unable to be opened upon initial test of door. ICLL(88)II/12(1)

Vessel does not have transfer procedures as required by 33 CFR subpart C.
