Install and operate a ballast water management system (BWMS) that has been approved by the Coast Guard under 46CFRpart 162. The BWMS must be installe in accordance with 151.2035(b) of this subpart. Following installation, the master, owner, operator, agent or person in charge of the vessel subject to this subpartmust properly maintain the BWMS in accordance with all manufacturer specifications. Vessels BWMS is inoperable.

In cargo pump rooms a system for continuous monitoring of the concentration of hydrocarbon gases shall be fitted. Fixed gas detection for the pump room was found by the PSCO to be alarming due to a flow-fault failure that was preventing the system from continuously monitoring the atmosphere in the pump room. 74 SOLAS (09), Chapter II-2, Regulation 4.5.10.1.3

In tankers, cargo ships shall be fitted with temperature sensing devices for bulkhead shafts, nearings, and pump cases. While conducting the pump room exam, the PSCO found the lower bearing temperature sensor on the number 3 cargo pump disconnected and not able to measure bearing temperature. 74 SOLAS (09), Chapter II-2, Regulation 4.5.10.1.1
