Vessel is required to ensure the EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. Vessel's EPIRB battery was past its expiration date. Replace battery or get new EPIRB to satisfaction of attending Coast Guard Marine Inspector. Vessel restricted to within three mile of shore. 46 CFR 185.728

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's fuel shutoff valve was not operable. Provide operational fuel shut off. 46 CFR 182.455(b)(4)

Where a fixed gas fire extinguishing system is installed in a space, all powered exhaust blowers for the space must be automatically shut down upon release of the extinguishing agent. Blower did not shut down when testing system. Provide means for blower to automatically shut down upon release of agent. 46 CFR 182.460(d)

Exhaust blower switches must be located outside of any space required to be ventilated by this section, and must be of the type interlocked with the starting switch and the ignition switch so that the blowers are started before the engine starter motor circuit or the engine ignition is energized. A red warning sign at the switch must state that the blowers must be operated prior to starting the engines for the time sufficient to insure at least one complete change of air in the space served. Blower not used prior to staring. Provide interlock so blower is operational for one complete air change prior to starting. 46 CFR 182.460(e)
