PSCO's observed the fuel line broke to the rescue boat engine and crew was not conducting/recording weekly inspections. 74 SOLAS(20) III/20.6.2 Code 17

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces, and other hazards. Observed lube oil leaking from STBD main propulsion engine from the seal of the connected hydraulic steering pump. 74 SOLAS(04) II-1/26.1 Code 17

A main source of electrical power of sufficent capacity to supply all those services mentioned in regulation 40.1.1 shall be provided. This main source of electrical power shall consist of at least two generating sets. PSCO observed 1 of the 2 main DGs inop. Vessel in conjunction with the Flag placed a portable power pack onboard and implemented mitigation measures. 74 SOLAS (04) II-I/41.1.1 Code 16

Cargo ships shall carry at least one rescue boat complying with the requirements of section 5.1 of the Code. PSCO observed an inoperable search light and missing thermal protective aids as required in all rescue boats. In addition, the inflated rescue boat is missing the required repair kit. 74 SOLAS (20) III/31.2 LSA 5.1.2.2 LSA 5.1.2.3 Code 16

Ad adequate system of ventilating crew accomodation shall be provided. PSCO observed inoperable ventillation in the water closet on the main deck. 46 USC 70002 ILO 147, pg. 33 Code 16

General arrangement plans shall be permanently exhibited... showing clearly for each deck the control stations, the carious fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions... Plans... shall be kept up to date; any alternations thereto shall be recorded as soon as practicable. PSCO observed the two doors in the galley as well as the both doors in the stairwell between the main deck and the accomodation deck do not meet the "A" class boundary requirements as prescribed by the ship's fire control plan. 74SOLAS(20) II-2/15.2.4.1 Code 50

... launching and embarkation appliances complying with the requirements of section 6.1 of the Code shall be provided for all survival craft...PSCO observed that the rescue boat davit does not have a limit switch as required. 74 SOLAS (20) III/16.1 LSA 6.1.2.7 Code 50

It shall be possible to control the propulsion machinery locally, even in case of failure in any part of the remote control system. PSCOs observed local steering control on port azipod not operable. 74SOLAS(20) II-1/31.2.6 Code: 17bc

Where air pipes to ballast and other tanks extend above the freeboard or superstructure decks, the exposed parts of the pipes shall be of substantial contruction. PSCOs observed port side air pipes to fuel oil tanks to not be of substantial construction due to allision in last port. ICLL 66 Annex I/Reg.20

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. Due to the objective evidence in the below deficiencies, the vessel is not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel’s SMS under the ISM code. PSCOs collected objective evidence that the vessel could not produce evidence that the company notified Flag Administration of the serious safety deficiencies, IBS acted on behalf of flag without administration approval. A certificate called a Cargo Ship Safety Construction Certificate shall be issued after an initial or renewal survey to a cargo ship which complies with the relevant requirements of Chapters II-1 and II-2 (other than those relating to fire safety systems and appliances and fire control plans) and any other relevant requirements of the present regulations. PSCOs found SOLAS Safety Construction Certificate expired on January 20, 2025. 74 SOLAS (2020) I/12(a(ii) Code 30bc

PSCOs observed an unauthorized power pack with portable fuel tank on the main deck. The crew stated the power pack is used as the vessel’s main service generator while in port. 74 SOLAS (2004) II-1/41.1.1 Code 17bc

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCOs observed portable power pack from main deck feeding a live cable to the main service electrical swith board. 74 SOLAS (2004) II-1/45.1.3 Code 17bc

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed broken piping on port side main deck exposing electrical wiring. 74 SOLAS 04 CH II/1/45.1.3

Electrictal services essential for safety will be ensured under various emergency conditions. PSCO observed broken latch used for operating ventilation in the emergency diesel generator space. 74 SOLAS 04 CH II-1/20.2

"A minimum of two inspections of the outside of the ship's bottom during any five-year period except where regulation 14(e) or (f) is applicable, this five-year period may be extended to coincide with the extended period of validity of the certificate. In all cases the interval between any two such inspections shall not exceed 36 months;" PSCO observed last two inspections of the ship's bottom documented on the cargo ship safety construction certificate are 22MAY2020 and 16JAN2024, thus not meeting the requirement to be within 36 months of each other. 74SOLAS(20)Ch.I,Reg.10(a)(v)

Ships shall be provided with independently driven fire pumps as follows: in cargo ships of less than 1,000 gross tonnage at least two power driven pumps, one of which shall be independently driven.PSCO's observed emergency fire pump was not bulding pressure.

Adequate information shall be provided to enable the equipment to be properly maintained, operated taking into action the recommendation of the organization. PSCO's observed the GMDSS log book didnt have entries sincde May 2024.

A duplicate set of fire control plans or a booklet stored in a prominently marked weathertight enclosure outside the deckhouse for the assistance of shore- side fire fighting personnel. PSCOs observed no fire control plan stored in a weathertight enclosure outside the deckhouse. 74 SOLAS (04) II-2/15.2.4.2

Machinery spaces of category A containing internal combustion machinery shall be provided with one of the fixed fire-extinguishing systems in paragraph 4.1. PSCO's observed two internal combustion engines located outside of a category A machinery space. That are used for the main propulsion of the vessel. PSCO's observed no fixed fire-extinguishing system and no approved alternative design or arrangements.

Every inflatable life raft...shall be serviced at intervals not exceeding 12 months... PSCO's observed the rescue boat and all 04 liferafts have not been serviced in over 12 months. All servicing is expired. 74 SOLAS (20) III/20.8.1.1.

Any main and auxiliary steering gear control system operable from the navigation bridge shall comply with the following... The system shall be capable of being brought into operation from a position on the navigation bridge... PSCO's observed starboard main diesel engine/ steering drive gear inoperable from the navigation bridge. 74 SOLAS (20) II-1/29.8.2.

The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following ballast water management methods: install and operate a ballast water management system that has been approved by the Coast Guard.... Use only water from the United States public water system...Perform complete ballast water exchange in an area 200NM from any shore prior to discharging ballast water...Do not discharge ballast water into the waters of the United States...Discharged a facility onshore or to another vessel for purposes of treatment.... PSCO's observed expired ballast water management certificate with inaccurate treatment methods. 33 CFR 151.2025

...Maintain a ballast water management plan that has been developed specifically for the vessel and that will allow those responsible... To understand and follow the vessel's ballast water management strategy... PSCO's observed vessel ballast water management plan was not specific to the vessel and did not accurately reflect ballast water treatment plan on board. 33 CFR 151.2050 (G)

Accumulator batteries shall be suitably housed. PSCO's observed accumulator battery in emergency fire pump room on forward port side was not housed. 74 SOLAS (20) II-1/45.9.1.
