All equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Each unpressurized fuel tank must be fitted with a vent pipe and the discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters.
Vent pipe on vessel was broken, and air was able to bypass the flame screen installed at the end of the vent opening.
