The side ropes of the embarkation ladder shall consist of two uncovered manila ropes not less than 65 mm in circumference on each side. All embarkation ladders were covered in vicicity of eye splice. Foward starboard ladder for 6-man liferaft found wasting under cover.

Vessel equipped with ballast tanks that operates on waters of the U.S. must employ an approved ballast water management methods, which does not include 200 nautical mile exchange when the vessel is required to employ an approved ballast water management system. Vessel has approved BWMS installed at last dry-dock in December 2020, and did not properly treat 21,500m3 loaded into #6 cargo hold. 11,885m3 of improperly treated ballast water discharged from #6 cargo hold.
