The original COI must be framed under glass or other transparent material and posted in a conspicuous place onboard the towing vessel. If posting is impracticable, the COI must be kept on board in a weathertight container and must be readily available. When the inspector requested the COI the master went to the Helm program and opened a scanned copy on the computer. Properly post the COI to the satisfaction of a USCG Marine Inspector. Original COI could not be found. USCG MSD Nashville has mailed a new COI to be posted.

The casualty was not reported and approved procedures were not followed. The master onboard was not aware of policy’s and procedures to follow for a Marine Casualty to include TVA’s policy for drug and alcohol testing for a Marine Casualty. Provide evidence that crew is properly trained on Marine Casualty reporting and drug and alcohol testing to the satisfaction of a USCG Marine Inspector.
