The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crewmembers are capable of performing the duties expected of them during emergencies. Crew could not conduct drills due to air starter on stbd engine not operating. Marine inspector must witness drills prior to operation.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Air starter inoperable when starting starboard engine. Diagnose cause of inoperable air starter and plan for repairs.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. RPM, oil pressure, jacket water alarms not communicating with pilot house if there were an emergency. Diagnose cause of inoperable alarms to pilot house and plan for repairs.

A fixed fire pump must be capable of delivering water simultaneously from the two highest hydrants, or from both branches of the fitting if the highest hydrant has a Siamese fitting, at a pitot tube pressure of at least 50 psi and a flow rate of at least 80 gallons per minute. The fire pump gauge is broken and fire main is leaking. Must have a way to verify psi and secure leak on fixed fire pump.

Vessel reported a failure to the starboard main engine resulting in a loss of propulsion. Troubleshoot and diagnose cause of failure to the satisfaction of attending USCG Inspector prior to further towing operations.

Alarms must be visible and audible at each operating station. The alarm located at the operating station may be a summary alarm; if the alarm at the operating station is a summary alarm, the specific alarm condition must be indicated at the machinery or bilge location. The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Observed wasted and missing pieces of rubber seal on watertight door leading from main deck to galley. Replace damaged seal.46CFR140.610(b)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Openings access to wiring within the distribution panels were observed in the wheelhouse and storage room distribution panels, install covers.

If the Master or Officer in charge of a Navigational watch believes it is unsafe for the vessel to procede, that an operation endangers the vessel or crew, or that an unsafe condition exist, he or she must ensure that adequate corrective action is taken and must not proceed until it is safe to do so. 46 CFR 140.210(b) Vessel allided with M/V MARTHA IVO Convent Marine terminal. Master of the vessel is required to provide USCG with statement attesting to: no damage occured to vessel or the barges, that a nav assessment was completed and that he feels the vessel is safe to transit for the intended voyage. THe vessel is also required to provide the USCG with a copy of the Marine surveyor report, before or when vessel arrives at MM 120.
