Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Communications Act Radiotelephony Certificates may be obtained from the Commission's National Call Center—[phone withheld]—or from its forms contractor. Radiotelephony certificate has expired. 47 CFR 80.903

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emrgency. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] to outboards must be approved by the OCMI. 46 CFR 182.200

Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. Port forward bilge pump light at operating station was not working due to a wire being disconnected under console.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit missing expiration dates on the outside of watertight container.

The following tests and inspections must be carried out weekly on a vessel: (a) Each survival craft, rescue boat, and launching appliance must be visually inspected to ensure its readiness for use Weekly inspection were not logged for visual inspection of life raft. A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Vessels alco test strips are expired, renew expired test strips. 46 CFR 185.212

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Non combustible garbage cans are missing lids. Replace missing non combustible garbage can lids. 46 CFR 177.405(f)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB hydrostatic release has expired. Renew hydrostatic release. 46 CFR 180.64

Public address systems.(a) Except as noted in paragraphs (d) and (e) below, each vessel must be equipped with a public address system.(b) On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members.(c) A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station.(d) On a vessel of not more than 19.8 meters (65 feet) in length, a battery powered bullhorn may serve as the public address system if audible throughout the accommodation spaces of the vessel during normal operating conditions. The bullhorn's batteries are to be continually maintained at a fully charged level by use of a battery charger or other means acceptable to the cognizant OCMI.(e) On a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, a public address system is not required if a public announcement made from operating station without amplification can be heard throughout the accommodation spaces of the vessel during normal operating conditions, to the satisfaction of the cognizant [name withheld] bow PA speakers on the main deck are not working. Repair or replace faulty speakers. 46 CFR 184.610

Vessel is required to maintain all portable fire extinguishers. Annual servicing not completed. Expired 2/22. 46 CFR 176.810

Vessel Captain and Crew need to undergo pre-employment drug testing and enroll in a drug testing program. 46 CFR 16.210 & 16.230

A mariner may not serve under the authority of an MMC past its expiration date. Chris Erickson has submitted paperwork to renew credential but needs to obtain a valid MMC. 46 CFR 10.205

The CG will issue a medical certificate to a mariner meeting the medical and physical standards for mariners. Vessel [name withheld], not able to provide medical certificate to attending MI. 46 CFR 10.301

Stability letter needs to be posted at the operating station. Not currently posted on the bridge.

Vessel shall label all fuel shut-off valves. 03 diesel tanks for heating are missing labeling. 46 CFR 185.608

Vessel is required to have 03 fire buckets. Vessel is missing 01 fire bucket. 46 CFR 181.610

Vessel owner shall display lifejacket donning instructions to passengers. Donning placard is missing. 46 CFR 185.516

Life jacket storage must be labeled showing type and number of life preservers. Missing labeling in the pilot house. 46 CFR 185.604(f)

Deck hatches shall be secured to the hull. Forward freeman hatches are not secured to the hull. 46 CFR 179.360(b)(2)

Vessel is required to have 01 ring life buoy light. Light is inoperable because the battery is missing. Must be fixed prior to night operations. 46 CFR 180.70(d)

Vessel is required to have 01 ring life buoy that is properly labeled. The ring life buoy was missing the vessel name. 46 CFR 185.604(b)

Vessel shall have up-to-date navigation publications and charts. Missing tide and current tables. 46 CFR 184.420
