Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the [name withheld] the portable fire extinguisher in a clearly visible and readily accessible location utilizing the manufacturer's provided mount.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Draft and post the broadcast instructions and information in the vicinity of the radio installation. Sample verbiage can be found in 46 CFR 184.510.

The construction of lights and shapes and the installation of lights on board the vessel must satisfy the Commandant, U.S. Coast Guard. Starboard sidelight is not USCG Approved. Replace with USCG Approved version. Vessel is limited to operating between sunrise and sunset (Daylight Operations Only) until repairs are completed.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The radiotelephone installation must be installed to insure safe operation of the equipment and to facilitate repair. It must be protected against the vibration, moisture, temperature, and excessive currents and voltages. Installed radio does not transmit. Repair as required.

Each vessel must— (1) Have permanent draft marks at each end of the vessel; or (2) Have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft. Loading marks required on hull are missing. Reapply as required.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jackets stored in same compartment as adults with no means of separation used. Separate child life jackets from adults as required.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. Except as otherwise noted, medical certificates will be issued for the following periods of time: Medical certificates issued to all other mariners will be issued for a maximum period of 5 years. Vessel's Captain has an expired medical certificate (Last issue 2016). Present Marine Inspector with objective evidence showing Captain has completed medical certificate requirements.

Each Employee Assistance Program education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crew member assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Vessel folder providing required elements of the EAP education program incomplete. Provide as required.

Each number required by 33 CFR 173.15 must: (1) Be painted on or permanently attached to each side of the forward half of the vessel except as allowed by paragraph (b) or required by paragraph (c) of this section; (2) Be in plain vertical block characters of not less than 3 inches in height; (3) Contrast with the color of the background and be distinctly visible and legible; (4) Have spaces or hyphens that are equal to the width of a letter other than “I” or a number other than “1” between the letter and number groupings (Example: DC 5678 EF or DC-5678-EF); and (5) Read from left to right. Vessel state registration numbers missing. Install numbering on hull as required.

Fuel fill pipes and sounding pipes must be suitably marked for identification. Fuel tank fill is not marked. Provide label "GASOLINE FILL" as required.

This vessel cannot carry passengers until it has completed the attached work-list, been re-inspected by Marine Inspectors assigned to the local cognizant OCMI in the vessel’s intended area of operation and issued a full-term Certificate of Inspection with conditions of operation.
