An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Employee Assistance Program (EAP) stated “Supervisors must be given sixty (60) minutes of training which is to be documented in the form of a log book entry attesting to the date/time, type of training (classroom, video, etc), and attending personnel. An entry is to be made in the Supervisor's personnel record that training has been accomplished.” Vessel could not provide satisfactory documentary evidence for required EAP training in accordance with SMS.

Internal combustion engines of 2,250 kW and above or having cylinders of more than 300 mm bore shall be provided with crankcase oil mist detectors or engine bearing temperature monitors or equivalent devices. Main engine’s ‘GRAVINER’ oil mist detector system had ‘COMMS FAULT’ and ‘DETECTOR FAULT’ on display. Crew continued to troubleshoot where they installed a spare electrical box PLC inside the control panel. Alarm faults cleared and retested system operation at oil mist detection # 1, 6, 2, and 9 all operational and satisfactory.

Emergency fire pump was leaking and the actuator for local start-up was taken apart and was under repair. The emergency fire pump can still be energized from the bridge and the Engine Control Room (ECR). Replace or repair in kind within 30 days.

Oily Water Separator valve position indicator sensor wire fell out of indicator housing, temporary repairs made. permanent repairs shall be made to the satisfaction of Class DNV within 30 days.

Marine Sanitation Device (MSD), noted the top of the MSD tank had severe wastage creating a through hole penetration into the MSD tank top. Temporary repairs were made with epoxy to close the penetration on the MSD tank top. Class DNV will issue a condition of class to submit and make permanent repairs to the MSD tank top within 30 days.

The Periodic Safety Test Procedures must be updated to include the newly installed Kongsberg Auto Chief 600 Propulsion Control System, this update to include all sub-systems for safety tests within the Periodic Safety Test Procedures, shall be submitted to DNV for review and approval within 90 days.

The condition of the ship and its equipment shall be maintain to conform with the provisions of the present regulations to ensure that the ship in all aspects will remain fit to proceed to sea without danger to the ship of persons on board. The ship reported that during discharge of cargo, the shore side gantry crane struck the ship's forward crane. The ship's forward crane will remain out of service until a damage assessment/inspection is completed to the satisfaction of the attending Class Surveyor prior to returning to service. 74 SOLAS(20)Chapter 1/11(a)

The company should establish procedures, plans, and instructions for key shipboard operations. Periodic safety tests required by 46 CFR 61.40-6 (see NVIC 01-13, Ch. 1) must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electric power generation and distribution, and emergency internal communications. Current DNV approved periodic safety test procedures (PSTPs) failed to provide enough detail to adequately test designed safeties (lack of alarm set point information, missing specific vital testing procedure details, and test procedures that were not specific to vessel equipment). PSTPs need to be updated, to ensure correct equipment and procedures are captured, and submitted for review. Prior to approval, PSTPs shall be demonstrated to the attending class surveyor for verification.

Vessel BWTS is inopt, vessel must conduct permanent repairs prior to bringing the system back in-operation. Vessel is to comply with the requirements of DNV condition of class and is authorized to discharge ballast so long as such water has been exchanged or taken from a distance of 200 NM from shore IAW 33CFR151.2025(a)(3)

A training manual complying with the requirements of paragraph 3 shall be provided in each crew mess room and recreation room or in each crew cabin; the training manual, which may comprise several volumes, shall contain instructions and information, in easily understood terms illustrated wherever possible, on the life-saving appliances provided in the ship and on the best methods of survival. Update vessel’s SOLAS training manual with instructions for operating new line throwing device and release hooks for lifeboats. Recommend including photos with captions on how to operate line throwing device and controls for releasing lifeboat hooks inside each lifeboat per OEM instructions.

Containers, brackets, racks, and other similar stowage locations for lifesaving equipment shall be marked with symbols in accordance with the recommendations of the Organization, indicating the devices stowed in that location for that purpose. The IMO placard for lifeboat located on port side main deck bulkhead outside of the lobby has peeled back and not displaying properly. Replace with new.

In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. Fire-fighter’s outfits shall comply with the Fire Safety Systems Code. Personal equipment shall consist of an axe (small fire axe) with a handle provided with high voltage insulation.

In addition to complying with specific provisions for fire integrity of bulkheads abd decks of cargo ships, the minimum fire integrity of bulkheads and decks shall be as prescribed in tables 9.5 and 9.6. A machinery space (6) adjacent to an accomodation space (3) shall be fitted with an A-60 bulkhead. Modification to the A-60 bulkhead separating the ship's laundry and machinery space at the main deck were made removing a section 200 cm by 200 cm A-60 bulhead. This modification was not approved by RO and not listed on the vessel's fire control or structural fire protection plan. Return bulkhead to original condition and insulate to meet A-60 standard. PUMS is restricted and a short term safety construction cert shall be issued by RO with a Condition of Class issued pending repairs to the satisfaction of attending surveyor [name withheld] once repairs are made to satisfaction of RO.
