On ships carrying flamable or toxic products, a water spray system for cooling, fire prevention, and crew protection should be installed. PSCO observed 13 DWS nozzles were not adequately covering tanktops 2,3,4 and both manifolds.

Fire-fighter's outfits shall comply with the fire safety systems code. Personal equipment shall consist of protective clothing of material to protect the skin from the heat radiating from the fire and from burns and scalding by steam. PSCOs discovered 04 fire-fighting jackets which were modified with stitching to hold the fire-fighting utility belts.

Each ballast water management system manufactured under coast guard approval must have an nameplate which is securley fastened to the BWMS and plainly marked by the manufacturer. PSCOs could not locate the BWMS nameplate. The crew searched for the BWMS nameplate until the conclusion of the exam with negativce findings.
