To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in § 176.500(b)(1) and your Certificate of Inspection must be endorsed. The owner did not complete the 2021 or 2022 Annual inspections. The vessel's COI was deactivated on this date. 46 CFR 176.502

New installations of electrical equipment and material must comply with 46 CFR 183. USCG inspector identified "new" electrical installations that do not match original approved plans for the vessel. The owner/operator installed three 12volt starting batteries for the port main engine. The original plans identify one 8D battery for engine starting. The new battery installation is wired in parallel using 8 AWG conductors between each battery. The owner/operator installed an additional 12volt battery in the port main engine compartment which is connected to a newly installed 12volt battery charger. The 12 volt battery charger was found connected to a portable extension cord that is installed through the aft bulkhead of the port main engine room to the aux generator space where it was found connected to a 3 outlet multi power strip, a 32 volt battery charger, and a second extension cord plugged into an AC receptacle outlet in the aux generator space. The battery chargers and new battery installations were not approved by the Coast Guard. If a battery bank is being used as a second power source for vital loads or as a final emergency power source, and is connected to a battery charger having an output of more than 2kW, it must be located in a dedicated locker, room, or enclosed box and electrical equipment inside must be rated for use in Class I, Div 1, Group B hazardous locations, and meet 111.105 in Subchapter J. It must be provided with a power exhaust ventilation system in accordance with 46 CFR 111.15-10. Verify that there is a power exhaust ventilation system on the one-line diagram, and that there is a comment to the effect that proper ventilation will be provided. USCG discovered a new microwave installation in the galley and several appliances located on the galley counter top to include a toaster and blender. The owner stated that the original approved 32 volt electrical system was removed from service but all conductors were left in place throughout the vessel which was last noted as a CG-835 in 2013 (MISLE Act# 4513736). A new 32 volt electrical battery installation was installed to power the toilet and several other items without plan approval. Based on the age of the vessel, the inspector was unable to determine the total amount of new electrical devices installed throughout the vessel. Submit a new one line electrical diagram and load analysis for AC electrical systems and DC electrical systems to the Marine Safety Center for review. Identify new electrical installations in the submission that differ from the original approved electrical plans. 46 CFR 183.115

Batteries used for engine starting are to be located as close as possible to the engine or engines served. The conductor attached to the positive starting battery terminal and starboard main engine starter was found installed so that it wraps around the battery tray into the bilge with a 180 degree bend back to the engine starter. The total length of this installation measured approximately 6 feet. The positive battery terminal measured less than 2 feet away from the starter. Shorten the distance for the positive conductor and contact USCG for inspection once installed. 46 CFR 183.350(h)

Batteries shall be located as high above the bilge as practicable, secured to protect against shifting, with the roll and pitch of the vessel, and free from exposure to water splash or spray. Batteries must be mounted in trays lined with or constructed of a material that is resistant to damage by electrolyte. Found new battery installation in the port main engine room. The battery is not installed to prevent shifting and placed directly against the hull without a tray. Install the battery so that it complies with 46 CFR 183.350.

Battery terminals shall be of the soldered type and all connections shall be made to the battery terminals with permanent type connectors. Butterfly nuts found installed on the stbd main engine starting battery terminals. Install permanent type connectors. 46 CFR 183.05-20(c)

Provisions shall be made by means of plugs, covers, etc. to prevent the admission of air in the space protected by a fixed fire extinguishing system. The operator closed the fire dampers in the port and starboard engine room. The dampers do not prevent the admission of air in the closed position. The dampers provide an approximate 2 inch gap between the rubber material used as a damper and the vessel's hull material. Arrange the engine room fire dampers to prevent the admission of air into the space. 46 CFR 181.20-35

All wiring shall be run as high as practicable above the bilges. Wiring joints and splices shall be mechanically secure and made in junction boxes. Identified several conductors installed in the port and starboard bilge. Identified DC and AC conductors spliced in the engine room and aux generator room without the use of a junction box and not mechanically secured. 46 CFR 183.05-50

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment. Identified green corrosion on the new conductor installed on the negative and positive terminals for the starboard main engine starting battery. Replace with conductors that comply with 46 CFR 183.340(d). 46 CFR 183.200(b)

Fuel system tanks shall be inspected by USCG. The fuel tanks are only partially visible from the top and cannot be visually inspected based on the original construction of the vessel. Submit a proposal to verify the fuel tanks are in satisfactory condition and can hold fuel without leaking. 46 CFR 176.25-10

On a non-metallic vessel, the enclosures and frames of electrical equipment must be bonded together to a common ground by normally non-current carrying conductor. Equipment grounding conductors must be sized in accordance with Section 250-96 of NFPA 70. The common ground was found heavily corroded in the auxiliary generator space. Replace in kind. Provide equipment grounding conductors to the common ground that comply with Section 250-96 of NFPA 70 once the common ground is renewed. 46 CFR 183.372 and 46 CFR 183.05-50(d)(2)

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding it designed limitations. The owner/operator has not implemented a maintenance program for the vessel's main engines. A maintenance schedule is not required by regulation. The engine manufacturer provided a recommended preventative maintenance schedule for the main engines and the operator has not implemented it. The inspector found no evidence of scheduled oil changes, raw cooling water impeller changes, or fuel filter changes, etc. in the vessel records or logs. The inspector discovered the raw water pump impeller fractured when the cover was removed by the owner voluntarily, at the owner's discretion, on the starboard main engine. The inspector also identified an aftermarket engine air intake filter attached to PVC piping on the port and starboard engines. The owner/operator shall provide evidence the "aftermarket" engine intake and filter are acceptable to the engine manufacturer or replace with a manufacturer approved intake/filter. Operators are responsible for maintaining their vessels accordingly. Replace the raw water impellers for the port/stbd main engines.

All fixed fire fighting systems shall be of an approved carbon dioxide type and installed to the satisfaction of the OCMI. Found the heat detector body located in the port and starboard engine room damaged. The hollow brass chamber was found inset which prevents the detector from meeting the original manufactured design to transmit air pressure increases through the copper piping. Replace in kind. Provide manufacturer instructions for the control head test and test in accordance with manufacturer's instructions. Provide manufacturer's instructions for a leak test on the copper tubing and complete the test in accordance with the manufacturer's instructions. Provide instructions for a functional test of the CO2 system with the bottles disconnected and complete the test in the presence of the attending marine inspector. 46 CFR 181.20-5

For purpose of flexibility only, flexible hose installed in lengths of 30 inches or less and subject to pressure less than 5 psi may be installed in vital fuel systems as long as they meet USCG Type A1, A2, B1 or B2 flexible hose. Identified flexible hose for fuel intake and return on the Aux generator less than 30 inches. The hoses were unidentifiable. Provide documentation that the hoses meet the USCG A1, A2, B1 or B2 standard or replace with approved flexible hose. 46 CFR 182.720(v)(B)

All electrical installations shall be to the satisfaction of the OCMI. Identified a fractured light housing in the stbd engine room. Replace in kind. 46 CFR 183.01-15

At each inspection the operator shall be prepared to conduct test and have the vsl ready for inspection including through hull fittings and sea valves. Through hull fitting in the port E/r was found broken and not watertight. Make ovbd fitting water tight on sea chest in port E/R.

Shut off valves must be installed at the tank and at the engine. No valve was found at the MDE side of the fuel lines on the port and stbd MDE.

Vessel did not have alcohol testing swabs on board. 46CFR4.06-3(a)(i)

Upper steering station on fly bridge has a hydraulic leak on the steering system. Vessel has ability to steer from steering station on main deck and fly bridge. Provide proof of proper operation of hydraulic steering system on fly bridge. 46CFR176.804

Vessel did not have MMSI number entered into radio. Provide proof of vessel's MMSI number correctly entered into radio. Vessel restricted to 1,000FT from shore. 46CFR184.502

A submersible electric bilge pump may be used as a power bilge pump, provided that: A positive shutoff valve is installed at the hull penetration. There are no shutoff valves installed on the overboard section of the submersible electric bilge pump piping for any of the 4 installed pumps. Install a shutoff valve at the overboard discharge of each bilge pump. 46CFR182.520(e)(7)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Non-metallic hoses on both the port and starboard engine's saltwater cooling system was bubbled and no longer holding its shape. Replace all deformed hoses in kind. 46CFR182.100

Except as otherwise provided in this section main propulsion machinery installations shall comply with the provisions of this part. MI observed soft patch on salt water supply line for the starboard engine. Replace in kind or provide a repair proposal. 46CFR 182.05-5 (a)

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Bile pumps in the starboard engine room and the aft compartment were inoperable. Repair or replace in kind both inoperable bilge pumps. 46 CFR 182.500 (a)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The visual and audible alarm for the aft space was inoperable. Repair to the satisfaction of the attending marine inspector. 46 CFR 182.530 (a)

Shutoff valves, installed so as to close against the fuel flow, one at the tank connection, shall be fitted in the fuel supply lines. Fuel shutoff valves are not easily accessible due to seized access cover. 46 CFR182.20-40(b)(3)

Shutoff valves, installed so as to close against the fuel flow, one at the tank connection, shall be fitted in the fuel supply lines. The shutoff valves could not completely close due to obstructions around the valves preventing them from turning 90 degrees.

In systems and applications where flexible hoses are permitted to be clamped, double hose clamping is required where practicable, the clamps must be of a corrosion resistant metallic material, the clamps must not depend on spring tension for their holding power; and two clamps must be used on each end of the hose, or one hose clamp can be used if the pipe ends are expanded or beaded to provide a positive stop against hose slippage. MI observed a missing hose clamp on the salt water cooling supply line for the port engine between strainer and engine. Additionally, the one provided hose clamp was not tightened around the metallic pipe allowing for a visible gap between the metallic pipe and the non-metallic hose. Add an additional hose clamp to this fitting and properly secure both clamps. 46 CFR 182.410 (d)

A vessel is inspected for compliance with the standards required by this subchapter. MI observed leak on port shaft seal during operation. Repair, tighten, or replace the packing for the shaft seal to secure the leak. 46 CFR 176.800 (a)

The part of the exhaust system between the point of cooling water injection and the engine manifold shall be water-jacketed or effectively insulated. There is missing exhaust insulation for the port engine before the cooling water injection point. Replace insulation. 46 CFR 20.15-15

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. The diesel fuel return line to the tank on the ship's generator was crimped and obstructing the flow of diesel fuel back to the fuel tank. Repair the line so there is no obstruction. 46 CFR 182.100

Fuel filters with non-metallic bowls must be fitted with flame shields. Fuel filters on main engines are missing flame shields. Provide flame shields. - 46CFR182.455(b)(6)

Provisions shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected. Starboard side engine room vent closure isn't closing completely. Repair. - 46CFR181.20-35(a)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Dead end wires discovered on port side aft cabin. Cap or remove. - 46CFR183.200(b)

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: "Emergency Exit, Keep Clear" Emergency exit in aft cabin isn't marked accordingly on either side. Provide Markings. - 46CFR185.35-15(a)

Vessel must complete a satisfactory dry dock exam or reciev an approved dry dock extension prior to carrying passengers IAW 46 CFR 176.600

05 Adult PFDs have degraded retroreflective tape. Replace wasted tape. 46 CFR 185.604 (h)

Vessel has no emergency broadcast placard posted next to radio installation. Install placard including vessel specific information in vicinity of radio installation. 46 CFR 184.506

Engine room forward bulkhead penetrations are not watertight. Make watertight. 46 CFR 179.320 (c)
