Machinery and fuel tank spaces must be separated from accommodation spaces by bulkheads. Bulkhead separating engine room from lower bunk room found to have holes at the base of the stairs, next to the thru bulkhead piping. Repair holes to the satisfaction of the MI. 46 CFR 144.410

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Measures must be in place to prevent flammable or combustible liquid piping leaks from coming into contact with these components. Gaps in lagging found on underside of exhaust piping above both main engines. Cover/secure gapping to MI satisfaction. 46 CFR 143.220(b)

Every towing vessel of at least 8 meters (26 feet) in length, measured from end to end over the deck (excluding sheer), must be under the direction and control of a person holding a MMC endorsed as master or mate (pilot) of towing vessels or as master or mate of vessels of greater than 200 gross register tons, holding a completed Towing Officer Assessment Record signed by a designated examiner indicating that the officer is proficient in the operation of towing vessels upon the appropriate route. Vessel did not have a Master endorsed for towing vessels. Vessel to provide a correctly endorsed Master to complete the underway portion of the exam including machinery / steering checks and drills to the satisfaction of the marine inspector.

Stations in the maritime service must be licensed by the FCC either individually or by fleet. One ship station license will be granted for operation of all maritime services transmitting equipment on board a vessel. Vessel was missing their Station License. 47 CFR 80.13(a)(b)

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. A navigation assessment entry must be recorded in the official log. Vessel was missing a navigational assessment.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weathertight function properly. Vessel must make engine room hatch weathertight prior to conducting operations.

The following alarms must be provided: main engine low oil pressure, main engine high water temperature, Aux generator high water temperature, and aux generator low oil pressure. Vessel must demonstrate working of alarms prior to conducting operations.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for intended voyage. Vessel had no publications or charts.

One ship station license will be granted for operation of all maritime services transmitting equipment on board a vessel. Vessel did not provide FCC Station License. 47 CFR 80.13(b)
