07109 - A towing vessel that is certificated for oceans must be equipped with either an approved 160-B semi-portable fire-extinguishing system to protect the engine room or a fixed fire extinguishing system installed to protect the engine room. Vessel does not have a 160-B extinguisher or authorized fixed fire-extinguishing system as defined in 46 CFR 136.110. Must be installed prior to towing operations. 46 CFR 142.315(a)(3)

07106 - Each towing vessel must have a fire-detection system installed to detect engine room fires. A towing vessel whose construction was contracted for prior to January 18, 2000, may use an existing engine room monitoring system (with fire-detection capability) instead of a fire-detection system, if the monitoring system is operable and complies with paragraphs (a)(2) through (7) of this section, and uses detectors listed by an NRTL. The vessel has an existing engine room monitoring system but the design and its installation has not been certified/inspected by a P.E., NICET IV fire alarm engineering technician or authorized classification society as per 46 CFR 142.330(a)(7). 46 CFR 142.330(a)(8).

07199 - Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. Vessel does not have a designated storage room or cabinet meeting the criteria in 142.225(c) on board the vessel. 46 CFR 142.225(a).

08199 - Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: Low hydraulic steering fluid levels. Vessel had a low hydraulic level alarm but it was not connected to the alarm panel on the bridge. 46 143.230(a)(6)

10109 - The sidelights of vessels of 20 meters or more in length shall be fitted with matt black inboard screens and meet the requirements of § 84.15. The screens on the vessel's sidelights were not painted matt black. 33 CFR § 84.05

11108 - Survival craft must be stowed in accordance with the requirements of § 199.130 of this chapter, as far as is practicable on existing towing vessels. Each liferaft or group of liferafts must be arranged for float-free launching as per 46 CFR 199.130(c)(7). Vessel's liferaft hydrostatic release unit was rigged upside down, rendering the liferaft incapable of float-free launching. 46 CFR 141.310.

The remote start for the fire pump located on the bridge was not operational. Ensure the remote start can energize the fire pump.

Show proof that all crew members are enrolled in a CG approved drug testing program and has completed a satisfactory pre-employment test.

Must complete MOB, fire, abandon ship drills, and annual inspection (i.e. testing equipment, complete engine room inspection)

The installed VHF radio DSC capability was not programmed. Ensure DSC is programmed on VHF radio. Vessel restricted to no more than 1000 ft from shore until DSC is functional.

The SSB did not have DSC capability. Ensure the SSB has DSC capability. Vessel is restricted to no more than 20 NM from shore.

Vessel is required 6 day and 6 night flares. All flares were expired. Ensure vessel has 6 day and 6 night serviceable flares.

Update first aid kit.

The battery for the EPIRB was expired. Ensure the vessel has a serviceable EPIRB. Vessel restricted to no more than 3 NM from shore until EPIRB is serviceable.

Ensure all spaces accessed by the crew has interconnected fire detection (UL 217)

Develop safe handling procedure for the handling and stowage of hazardous items such as rechargeable batteries.

For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include, but need not be limited to— (1) Training in the use and location of firefighting equipment and general firefighting knowledge, including: (i) Location of firefighting appliances and emergency escape routes; (ii) Types and sources of ignition; (iii) Flammable materials, fire hazards and spread of fire; (iv) The need for constant vigilance; (v) Actions to be taken on board; (vi) Fire and smoke detection and automatic systems on board; and (vii) Classification of fire and applicable extinguishing agents. (2) The drills required by § 185.524, including fire location and fire type; and (3) Emergency egress training for each member of the crew, to occur for all members of the crew— (i) At least monthly while such members are employed on board the vessels; and (ii) Each time a crew member joins the crew of such vessel. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics.

Provide proof that 50% of the crew is first aid and CPR trained

Vessels over 40' are required to have a waste management plan.

Ensure there is updated Coast Pilot, Tides, and Currents onboard.

Ensure at least one ring life buoy does not have a line attached.

Ensure PFDs and immersion suits have serviceable water lights. Batteries have not been change annually.

Ensure there is a sign for the sewage discharge valve indicating that the valve shall not be opened within 3 NM from shore and ensure the valve is secure shut.

Two cans of AFFF were stowed on deck and frozen. Provide manufacturers instruction for stowage and servicing.

Ensure all e-lighting is operational.

A fuel leak from a fitting on the fuel tank was noticed in the machinery space. Repair the leak and clean the bilge of all fuel.

Ensure the hailing port is marked on the stern prior to completing drydock (31MAR23).

Vessel is required to have eleven (11) immersion suits. Vessel had nine (09) serviceable immersion suits. One suit was missing and one needs serviced or renewed. Vessel restricted to nine (09) total people until full complement of immersions suits are on board.

Port general alarm light out in engine room.

E-lights were not operational.

Multiple lights were extinguished.

Exhaust leak on port gen which included oil leak.

Ensure there is a sign at sewage discharge that states do not open within 3 NM of land.

Vessel required horn.

Provide to local [name withheld] office a shaft and rudder post report that attest's to the condition of fitness for continued service. This report should include visual inspection, dye testing results, and balancing measurements.

Provide adequate rudder angle steerage for the port rudder. The port rudder does not acheive full rudder commands when placed in hard port. Rudder test to be witnessed by an attending CG Marine Inspector.

Conduct emergency fire fighting drill with crew.

Conduct emergency man overboard and abandon ship drills with crew.

Emergency training and drills must be conducted for each crewmember prior to getting underway for the first time on a particular vessel and at least once every three months. Company shall provide this office with documentation of this training/drills every quarter until next annual exam. June-2019, Sep-2019, Dec-2019, Mar-2020

Vessel must be able to shut down a propulsion engine from the pilothouse control station. Prove proper operation of port MDE shutdown from pilothouse.

Vessel shall test fire detection system in the presence of CG Marine Inspector or have an approved third party test and provide a report to this office.

Vessel shall have a Station Bill that is vessel specific onboard and posted in a conspicuous location.

Company shall provide an Employee Assistance Program for all crewmembers to include a community service hotline telephone number for crewmember assistance.

Vessel must carry a first aid kit that contains the contents required in 46CFR160.041.

Vessel must carry current navigation publications to include a Coast Pilot and Light List. (extracts of areas of operation are acceptable)

A vessel over 65ft must have a fixed public address system and be audible during normal operating conditions throughout accomodations spaces and all other spaces normally manned by crew members.

Vessel's rudder must be capable of moving from 35 degrees on one side to 30 degrees on the other side in not more than 28 seconds with vessel moving ahead at maximum service speed.

Vessel shall obtain a Vessel General Permit (VGP) from EPA and follow the requirements of the Permit, which includes a Notice of Intent and Annual report sent to EPA. Section 312 of Clean Water Act CG-543 Policy Letter 11-01

An electrical distribution panel shall be labeled and each unused slot shall be covered to prevent a person from accidentally contacting energized parts.

Bilges shall be free of excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, or clog bilge pumping systems.

Vessel must have adequate emergency lighting for a crew member to escape to the main deck. Engine room had four emergency lights that were not operational.

Crop and Renew to good metal - forward outboard bulkhead in Ballast #2 stbd near hydraulic lines.

Crop and renew to good metal - inboard bulkheads of voids under port and starboard 'cargo' tanks.

Pressure test port and starboard Voids #1 to verify welds on bottom hull are satisfactory while on drydock.

The construction and arrangement of a vessel must allow the safe operation of the vessel IAW COI giving consideration to provisions for a seaworthy hull, guards and rails. Vessel had starboard bit and bulwark ripped off while in tow. Vessel shall make satisfactory repairs to maintain structural integrity.

Company failed to notify of marine casualty.

Master of the vessel did not immediately report damage to the vessel (reportable marine casualty).

Stow all life jackets so that they are readily available and can be donned without removing buckles or straps.

ACR strobe lights found on 1 PFD and 2 immersion suits need to have lithium batteries installed.

Replace battery in both Port and STBD upper deck life buoy float lights with approved batteries.

Replace type II retro-reflective material on Port upper deck life buoy and port and stbd forward life buoy's.

Make repairs and perform leak test or replace damage immersion suit to left hand.

Make repairs to emergency lighting in engine room (port aft and stbd fwd).

Make repairs to Bus C ground detection panel.

Make repairs to fire pump to produce a minimum of 50gpm at 60psi.

Prove proper operation of both bilge pumps.

Make repairs to radar.

Provide vessel VGP.

Provide alcohol test strips for full crew.

Provide employee assistance program.

Make repairs to VHF. VHF would not transmit on CH 16.

Make repairs to the port rudder angle indicator on bridge.

Provide proper number of life jackets with lights as per COI and properly mark with vsl name on each.

Provide proper amount of immersion suits with lights as per COI and properly mark with vsl name.

Make repairs to heat detector in pump room.

Pump out / clean oily water in engine room bilge.

Make repairs to port engine shutdown on bridge.
