Resolution: VESSEL REPLACED BOTH CURRENT TRANSFORMERS, TESTED BOTH OVERCURRENT PROTECTIONS, AND PROVIDED PROOF OF RECTIFICATION ALONG WITH MASTERS ATTESTATION OF REPAIRS.
The condition of the ship and its equipment shall be maintained to conform with the provisions of regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed flange between OP-250 and OP-256 valves leaking crude oil cargo. This flange is in line with Cargo Pump #2 at bottom of Cargo Pump Room. Leak has been determined to be from expansion due to temperature and it is estimated to be approximately 0.2 m3 that leaked from flange. OP-250 is a non-return valve and was closed due to cargo operations being secured prior to PSCO attendance. Vessel is restricted from using Cargo Pump #2 for cargo operations.
