In a passenger ship, the required fire pumps shall be capable of delivering for fire fighting purposes a quantity of water, at the appropriate pressure prescribed. During the operational test of the general service fire pump in the engine room, the pump discharge piping ruptured rendering the pump incapable of providing pressure to the fire main system. PSCO attempted to conduct operational test of the secondary fire pump located forward of the engine room. The secondary pump was also unable to provide pressure to the fire main system. The two way valve that diverts discharge from each fire pumps discharge piping to the fire main system was seized causing the secondary pumps discharge back to the general service fire pumps ruptured piping in the engine room.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. In meeting there requirements the Company should ensure that appropriate corrective action is taken. Life saving equipment throughout the ship is not maintained or serviceable. Corrective action has not been taken. SOLAS 74 (14 CONS), CH IX REG 3.1 / ISM CODE 10.2.3

After any survey of the ship under Regulations 7, 8, 9 or 10 of this Chapter has been completed, no change shall be made in the structural arrangements, machinery, equipment, etc. covered by the survey, without the sanction of the Administration. The main propulsion engines and associated propulsion equipment have not been maintained to ensure the safe operation of the ship. During operational test of the starboard main propulsion engine, the PSCO witnessed the loss of lubricating oil requiring the engine to be secured due to gauge failure. It was reported to the PSCO that both main propulsion engines and reduction gears routinely overheated, requiring the crew to reduce engine load and alternate between reduction gears during the vessels transit from Indonesia to Guam.

Each administration shall require its ships to be manned in compliance with the applicable safe manning requirements. The ship was not in compliance with safe manning requirements. The ship was missing one Rating in charge of Navigation watch II/4, and one rating forming part of an engine room watch III/4 as required by their minimum safe manning document.

An International Load Lone Certificate shall be issued to every ship which has been surveyed. The Ship does not have a valid Certificate. The certificate expired on 02/09/2021. ICLL 66 ARTICLE 15

A certificate called a Passenger Ship Safety Certificate shall be issued after inspection and survey to a passenger ship which complies with the requirements of Chapters II, III, and IV and any other relevant requirements of the present regulations. Vessel’s Passenger Ship Safety Certificate was issued to expire after a single voyage from Benoa, Indonesia to dock in Guam.
