Each vessel that relies on electrical power must be arranged so that the loads can be energized from 2 sources of electricity: vital systems, interior lighting, comms systems including PA system, navigation equipment and lights.
Vessel did not have back up power for PA system or navigation systems and lights. Vessel shall install back up power for PA system and navigation system and lights that will maintain power for 03 hours after vessel loses ship's power.

At inspection for certification of a vessel, owner /operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems.
Vessel's engine rooms contained excessive oil on deck and a shaft seal was noticed to be worn out, allowing water intake. Inspection of all machinery spaces and units were halted. All oil, water, known damage to machinery and cause of spill are to be properly packaged, offloaded, cleaned, repaired and made ready for operation and inspection. No operations of machinery will be allowed and vessel is to remained tied to the pier until satisfaction of Marine Inspector.

The vessel recently changed managing operator and inspection could not determine when workboat's lifting strap was inspected. Lifting strap's condition was questionable with mold and UV damage. The majority of the retroreflective material on both lifefloats were worn with some areas covered with paint overspray rendering its ability to function as necessary.
