Fire Pump shall be 50 gallons per minute at a pressure of not less than 60 PSI at the pump outlet. During fire drill, fire pump was displaying 42 psi. Repair/adjust submersible fire pump to output 60 psi from gauge or nozzle.

Electrical equipment vital to safe navigation will be given special consideration shall be subject to req. from OCMI; Each vessel that relies on electrical power must be arranged so that the loads can be energized from 2 sources of electricity: vital systems, interior lighting, comms systems including PA system, navigation equipment and lights. Vessel did not have back up power for PA system or navigation systems and lights. Vessel shall install back up power for PA system and navigation system and lights that will maintain power for 03 hours after vessel loses ship's power.

Marine Inspector shall require that all observed unsafe practices and hazardous situation be corrected. Emergency lights shall be in each accommodation space (01 room did not contain an emergency light) and the lights shall be directed towards each exit of the space (port aft room emergency light was not directed toward aft exit). Cite: 46 CFR 176.25-50 OLD T

Fuel lines must be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration. Fuel line in machinery space noted to not have chafing gear. Vessel shall install with chafing gear on fuel lines.

Hatches must be made weathertight. Hatch going from accommodation space to engine room was noted to utilize a non marine grade gasket. Vessel shall install a weathertight, marine grade gasket.

Cable and wires must be installed with metal supports spaced not more than 24 inches apart and in a manner to avoid chafing. Vessel noted to have metal support on one wire for interconnected fire detection but plastic ties for the rest. Wires going throughout the vessel must be install with metal supports (not metal or plastic ties, but supports that are with the overhead, a beam, etc).

The COI issued to a vessel describes the vessel routes, minimum manning requirements, etc. and any equivalencies accepted or authorized by the OCMI. Vessel went underway/in transit without meeting minimum manning requirements (logged 03 members when in transit when COI states minimum of 04). Vessel shall maintain minimum manning when in transit and any other manning requirements as stated on COI.

At inspection for certification of a vessel, owner /operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems. Vessel's engine rooms contained excessive oil on deck and a shaft seal was noticed to be worn out, allowing water intake. Inspection of all machinery spaces and units were halted. All oil, water, known damage to machinery and cause of spill are to be properly packaged, offloaded, cleaned, repaired and made ready for operation and inspection. No operations of machinery will be allowed and vessel is to remained tied to the pier until satisfaction of Marine Inspector.

Vessel must have an interconnected detection and alarm system. Vessel is to install interconnected fire and smoke detection system and the appropriate type of detection system in specific location as discussed.

A visual and audible alarm must be provided at the operating station to indicate a high water level in bilges. No switch is permitted to silence or disable any functions of the high bilge alarms. Switches that have the capability to do so are to be disconnected.

For intended voyage, vessel must carry adequate and up-to-date charts of large enough scale for safe navigation. Chart corrections must be recorded and performed so all charts to be up-to-date.

A person may not employ or engage in a position required to hold an MMC unless the individual maintains a current medical card. Licensed mate is to have a valid medical card and TWIC with them while onboard and engaged in operations and inspections.

Crew accommodation and work space must be with suitable equipment for the protection and accommodation of the crew while in modes of operation of the vessel. Directv box was held up by a rubber strap, must be placed in a secured location to prevent injury of crew.

Each survival craft equipment must be of good quality and efficient for it purpose. Both lifefloat's plugs were puncture/damage and will need to be repaired/replaced to be in original factory condition.

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use. Retro reflective tapes must be replaced on all lifefloats, rescue boat including an additional strip on the bottom side, and life rings.

All electrical installations shall be to the satisfaction of the OCMI; wiring for submersible pump was frayed and spliced without proper covering. Replace submersible pump prior to departure. 46 CFR 183.01-15(a) Old "T"

Vessels with power-driven fire pump shall also be provided with fire main system; fire main system in deficient condition, pipe leaked when running due to heavy corrosion. Replace piping prior to departure. 46 CFR 181.15-1 Old "T"

Scantlings and material of each vessel are to be such as to provide for safe operation; ballast water vent had significant corrosion, compromising watertight integrity of vessel. Renew vent. 46 CFR 177.01-5(a) Old "T"

The inspection and maintenance of launching appliances shall be logged or otherwise documented for review by the Coast Guard upon request. The vessel recently changed managing operator and inspection could not determine when workboat's lifting strap was inspected. Lifting strap's condition was questionable with mold and UV damage. If replaced, strap shall have a design safety factor of 6 times maximum working load. 46 CFR 185.702(d), District 8 Policy Ltr 98-13, Rescue Boat Requirements on OSVs.

Each hatch used as a means of escape, must be capable of being opened by one person, from either side. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Engine room escape hatch spring had wasted and broke making hatch more difficult to open from below deck. 46 CFR 177.500(h)

A vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have a mechanical means of closing each drain and scupper in the container or enclosed deck area. Vessel's fuel vent containments and deck scuppers were missing closure devices. 46 CFR 184.702, 33 CFR 155.310 (a)(3)

At each initial and subsequent inspection for certification, re-inspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Port engine fuel return line was discovered leaking diesel fuel above engine at shut off valve between flexible hose and hard piping shut off valve. Vessel shall secure fuel leak prior to operating port engine. 46CFR176.25-50(a)

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition and fit for the service for which they are intended: Engine starting system. Alternate methods of starting shall be checked. Device/actuation spring on port and stbd engines used for operating station engine shutdowns was discovered altered with zip ties to function properly. Repair/replace as per manufacturer requirements. 46CFR176.25-10(a)(2)

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service for which they are intended: (4) Generators. All generators, motors, lighting fixtures and circuit interrupting devices located in spaces or areas which may contain flammable vapors shall be checked. Starboard generator would not start at time of inspection. Vessel was able to get generator to properly function prior to marine inspector’s departure from vessel. 46CFR176.25-15(a)

A vessel must have one or more ring life buoys as follows: (c) At least one ring life buoy must be fitted with a lifeline. The lifeline bag attached to two of the life buoys onboard had a zip tie to secure the lifeline from slipping out of the bag. The rendered the ability for the line to readily release from the bag. Vessel shall stow lifeline with a different arrangement which does not prevent the lifeline from being casted rapidly. 46CFR180.70(a)

All life preservers shall be of an approved type. Three lifejackets onboard had worn retroreflective material attached. Vessel shall replace retroreflective material to meet 46CFR180.25-25 or replace with new lifejackets. 46CFR180.25-1(a)

Each survival craft required on a vessel by this part must meet one of the following: (2) For a life float—Approved under approval series 160.027 or other standard specified by the Commandant. Each buoyant apparatus must have type II retroreflective material meeting subpart 164.018 of this chapter on each side and on each to be placed in accordance with this section. The majority of the retroreflective material on both lifefloats were worn with some areas covered with paint overspray rendering its ability to function as necessary. Vessel shall replace all retroreflective material. 46CFR180.200(a)(2), 46CFR160.027, 46CFR160.010-4(p)

Vessels Operations Manual requires actions based upon wind speed. Vessels anemometer would not read wind speed, only wind direction, at time of inspection. Vessel shall ensure a proper functioning anemometer is onboard prior to jacking operations. CG 543 Policy Letter 07-02 Enclosure (1)11.C

At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Oil was discovered pooled up at engine room bilge discharge suction box, leading to a future oil discharge if bilge pump was operated. Vessel shall remove all oil in box and around area which can lead to the accumulation.

Vessel is required to be equipped with an adequate rescue boat. During man overboard drill rescue boat engine would not operate and crewmember found excessive water in portable gas tank. New gas tank was provided and engine would still operate sporadically. Crew to service rescue boat and prove satisfactory operation to attending MI. 46 CFR 180.210 (b)(2),Correct prior to departure.

Each lifelfoat shall be equipped with two paddles. Paddles provided for both lifefloats were cracked from exposure. 46 CFR 180.15, correct in 7 days.

The stacked lifefloats are missing spacers between each other.

Draft markings are noticeably incorrect.

Generator space emergency light is inoperable.

Attached light is inoperable.

Stern light lense is covered in black paint overspray.

Dogs on watertight door on main deck were inoperable and need to be repaired or replaced.

Load mark was not on the hull.

Lifefloat lifeline was deteriorated and needs to be replaced.

Fire control plan is not current; it does not reflect the current layout of fire adn safety items onboard.

Upon Arrival in Venice, LA and Prior to placing the vessel back into opertion or departing Venice, LA. Contact USCG Sector New Orleans.

Lifefloat painter line catches in tightly packed coil. Replace coil to ensure readiness for float free arrangement.

Lines attaching waterlights to two (2) ring buoys decaying/not in a servicable condition.

Ventilation on port side of engine room wasted through to above the accomodation space. crop and renew to ensure continuity of barrier beteween engine and accomodation spaces.

Marine sanitation device missing approval placard and posted operating instructions.

Engine room distribution panel missing blank and directory needs to be updated.

Engine room escape hatch does not completely dog tight. Adjust dogs to ensure watertight enclosure.

Port generator inoperative. Show proper operation to attending marine inspector.

Shore tie breaker removed. Prove disconnection of or remove shore tie cable.

Paint locker equipped with fixed fire extinguisher that does not have a closure for the vent. Demonstrate ability to acheive vapor tightness.

Need to replace Electrical box in galley with a 4 outlet box to accomodate electrical equipment

Change gasket on Aft berthing WTD with correct hard rubber to ensure weathertight seal

Install plug for all deck combing to prevent pollution in water from hydraulic leaks from Hose/piping.

Install a cover for Engine Room Electrical Panel

Lock or remove valve handel on the overboard discharge pipe for the MSD

Replace light cover and grease screen for galley exhaust fan.

Label engine room vent with "Close vents in case of fire"

Replace Flame/Insect Screen on MSD gooseneck vent.

Adjust the Stbd rudder stop to ensure that RAM does not touch rudder stop at right full rudder.

Replace and renew fractured angle stiffener in Engine Room as new.

Prove proper repairs to handrails located behind sewage system.

Demonstrate compliance with the 8th U.S. Coast Guard District's Policy "Rescue Boat Requirements on Offshore Supply Vessels" dated 12NOV98 to the satisfaction of the attending Marine Inspector.

PROVIDE ON BOARD A COAST GUARD ISSUED STABILITY LETTER.
