Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. The port side fire damper did not create a vapor tight seal. Adjust and repair fire damper and make it vapor/gas tight.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2.5 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. The three fire buckets were 5 gallons each and were not labeled. Obtain the correct size fire buckets and label them.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The visual indicator for the bilge pump in the aft compartment was inoperable. Repair the indicator.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. 01 life float had fractures on the top surface. Repair the fractures with an approved repair kit or product according to [name withheld] manufacturer instructions.

Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel is constructed with general purpose resin. The under deck of the engine room was found lacking of non-conbustible panels or materials. Install fire retardant insulation or similar. Provide the specs of the materials to be used to the USCG for approval, and prior installation in the under deck of the engine room compartment.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. The engine room ventilation on port and starboard side closed but did not stay shut. Repair to make sure the ventilation can remain shut on both sides. 46 CFR 182.465(h) 17c

Each life jacket kept in a stowage container must be readily available. The life jackets were kept in zipped stowage bags that had to be ripped open to gain access to the life jackets. Rectified on the spot by vessel crew tearing open all bags. 46 CFR 180.78(a)(2) 17c

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts. Vessel Captain was unable to show inspection team how electronic chart receives updates. Demonstrate that the electronic chart receives updates and is up-to-date. 46 CFR 184.420(a)(1) 16c

A vessel of more than 26 feet must carry a minimum of one life buoy. The life buoy on board was waterlogged and needs to be replaced. Replace with new life buoy. 46 CFR 180.70(a)(2) 16c

Vessel made contact with an object has suffered bent propellers.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Provide a nautical chart with current updates. 46 CFR 184.420(a) 16C.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Replace (01) portable flashlight that was found inoperative. 46 CFR 183.430 16C.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Master and crew were found not to be completely familiar with current emergency instructions that needs to be updated in accordance with 46 CFR 185.512 recommended emergency instructions format, adapting it to what the current vessel's arrangements and equipment. 46 CFR 185.510 16C.

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following: (1) The location of emergency exits, survival craft embarkation areas, and ring life buoys; (2) The stowage location(s) of life jackets; (3) The proper method of donning and adjusting life jackets including a demonstration of the proper donning of a lifejacket, or (4) The location of the instruction placards for life jackets and other lifesaving devices; (5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master or crew. Crew members didn't met the requirements of the safety orientation when asked. Demonstrate proficiency in providing safety orientation complying with the requirements of 46 CFR 185.506 (above) and in presence of an attending USCG Marine Inspector. 46CFR185.512 16C.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel was pushed onto the beach while anchoring and damage was reportedly sustained to propellers and rudder. Vessel to be hauled out and inspected for further damage by marine inspectors. Repairs to be conducted to attending marine inspector satisfaction prior to returning to service. 46 CFR 176.840

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. COI was not posted. 46cfr176.302

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel missing FCC inspection.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Not posted at operating station. 46cfr184.506

Life floats and buoyant apparatus must not be stowed in tiers more than 1,220 millimeters (4 feet) high. When stowed in tiers, the separate units must be kept apart by spacers. Life Floats had no spacers. 46cfr180.137(f)

Unless otherwise stated in this section, life jackets must be stored in convenient places distributed throughout accommodation spaces. Each stowage container for life jackets must not be capable of being locked. If practicable, the container must be designed to allow the life jackets to float free. Each life jacket kept in a stowage container must be readily available. Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Life jackets required proper stowage area so marked. 46cfr180.78

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Battery installation was not secured to prevent movements but instead rested on top of aft steering space bilge pump. Mount properly. 46cfr183.350

Vessel requires inspectors to observe fire and man overboard drills. 46cfr176.840

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Container lacked labeling. 46cfr185.614

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. Plan not in place. 33cfr151.57
