Every line above is a count of records the US Coast Guard wrote, or a date it recorded. A blank resolution means none was recorded in this data; it does not establish that a deficiency is outstanding. Years with a record are calendar years in which a record exists, not years in service. 07 - Fire Safety › N/A - No Subsystem › Fire prevention structural integrity
Issued 24 February 2026 Resolved
Fire control bulkheads between various spaces must meet the requirements for structural fire protection insulation as such in Table 116.416(b). The main deck fwd laundry is a "High risk service space" that was missing required A-60 insulation on the entire aft bulkhead and missing a small section of insulation on the forward bulkhead. Install A-60 insulation to the satisfaction of the attending Marine Inspector.
Action required: 50 - Rectify deficiencies w/in 30 days
Resolved 1 May 2026
Resolution: Received a revised Fire plan approved by MSC that changed the forward main deck laundry to a low risk space. The space is now designated as an A-0 class fire boundary.
13 - Propulsion and Auxiliary Machinery › N/A - No Subsystem › Propulsion main engine
Issued 24 April 2023 Resolved
The port main engine's mounts had to be repositioned due to misalignment. The welds for the mount reinforcement plates to the deck have significant undercut throughout all four engine mount weldments. Also, significant cold-rolling was identified on the fwd-outboard and aft-inboard mounts.
Condition: Improper Installation
Action required: 16 - Rectify deficiencies w/in 14 days
Due 8 May 2023
Resolved 2 May 2023
Resolution: Welds on all mounts were repaired and are satisfactory.
09 - Working and Living Conditions › 092 - Working Conditions › Machinery
Issued 14 April 2023 Resolved
Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. On April 14, 2023, the vessel experienced a propulsion casualty to its port Z-drive. The [name withheld] has determined that the vessel in its current condition, with only one engine operational & redundant system OOC, poses a significant risk to the waterway and passengers without additional mitigation. The vessel is required to submit to this office a Transit Plan and receive approval prior to departure; the plan must include a towing vessel of adequate horsepower during all transits within the Lower Mississippi River; until the port engine has been repaired to the satisfaction of the attending Marine Inspector. 46 CFR 119.100 33 CFR 160.111 (c)
Condition: Invalid
Action required: 705 - Other - as specified
Due 14 April 2023
Resolved 24 April 2023
Resolution: Received, reviewed and accepted three technician reports from the vessel's company. See narrative in act# 7674910 for more information.
07 - Fire Safety › N/A - No Subsystem › Fire doors/openings in fire-resisting divisions
Issued 14 April 2023 Resolved
§ 116.435 (b)(7)Doors, Hose ports, if fitted, must be in the lower corner of the door opposite the hinge so a hose may pass through the doorway when the door is open and still allow the door to close over the hose. The hose port should be approximately 152 millimeters (6 inches) square. A self-closing hinged or pivoted steel or equivalent material cover must be fitted in the opening. During today's fire drill in the elevator shafts, no hose could reach the elevators without holding open the fire screen doors to the compartment. Since fire screen doors are required to be closed, hose ports are required in one of the fire screen doors on each deck level of the vessel where the elevator doors are.
Action required: 50 - Rectify deficiencies w/in 30 days
Due 17 May 2023
Resolved 13 July 2023
Resolution: 835 removed appeal granted not to install hose port doors.
11 - Life Saving Appliances › N/A - No Subsystem › Lifejackets incl. provision and disposition
Issued 14 April 2023 Resolved
The number and identification of the life jackets stowed inside, and their sizes, must be clearly marked. Passenger cabins do not have marking of where life jackets are stowed. Clearly mark passenger cabin life jacket stowage locations.46 CFR 122.604(f)
Action required: 10 - Deficiency Rectified
Due 20 April 2023
Resolved 31 August 2023
Resolution: Provided adequate storage and labeling location in passenger staterooms.
11 - Life Saving Appliances › N/A - No Subsystem › Lifejackets incl. provision and disposition
Issued 14 April 2023 Resolved
Life jackets must be stored in convenient places distributed throughout accommodation spaces. Life jackets in passenger cabins are currently stored under the passenger's beds. Due to the age of the average passenger, this is not a convenient stowage location for all passengers to have access to. Relocate passenger cabin life jackets to a convenient location for the passengers. 46 CFR 117.78(a)
Action required: 10 - Deficiency Rectified
Due 20 April 2023
Resolved 31 August 2023
Resolution: Provided adequate storage and labeling location in passenger staterooms.
