When transporting a solid bulk cargo which is liable to emit a toxic or flammable gas, or cause oxygen depletion in the cargo space, an appropriate instrument for measuring the concentration of gas or oxygen in the air shall be provided together with detailed instructions for its use. Such instrument shall be to the satisfaction of the administration. Manufacturer's requirements for the vessel's portable gas meters are required to be bump-checked with the recommended calibration gas prior to each use. The vessel has not have calibration gas on board the vessel since March 2025. Vessel is currently loaded with coal requiring gas detection and has loaded cargoes requiring gas detection since March 2025. Vessels safety management system does not list the meter found on board the vessel.

Where a solid bulk cargo is specifically listed in appendix 1 to this Code, it shall be transported in accordance with the provisions in its schedule in its schedule. All electrical cables and components situated in cargo spaces and adjacent enclosed spaces are free from defects. Such cables and electrical components are safe for use in an explosive atmosphere or positively isolated. The vessel's safety management system (4.1.3.10) states "It is essential that any fixed hold lightning system are isolated, or the fuses removed when the holds contain cargo." PSCO observed on the deck walk that the cargo hold lighting was not isolated. Crew isolated after being made aware of the situation. PSCO additionally observed that the securing of electricity to the holds is not addressed in Vessel Form Manual Section 7.
