The company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of personnel.
There was an incident/accident during pilot embarkation at Annapolis anchofage Maryland, USA that related to rigging of the pilot ladder/embarkation ladder arrangements.
At present the company's SMS has several periodical inspection/maintenance jobs for the pilot embarkation arrangement, but no procedure/checklist for the rigging of the pilot embarkation arrangements itself. The accident provides objective evidence of a need for a procedure to encure the safety of personnel. Investigate this accident per ISM A/9.1 and provide a corrective action plan including actions to prevent recurrence, to the satisaction of RO and CG.

A pilot ladder shall be certified by the manufacturer as complying with this section or with the requirements of an interational standard acceptable to the organization (ISO 799).
The pilot ladder currently in use on the starboard side from Qingdao Good Brother, serial No. P19330 on the data placard on the ladder itself, does not correspond with the manufacturer's approval certificate available onboard at time of inspection serial No. 32946. Remove this ladder from service and replace with onboard spare ladder until appropriate paperwork with matching serial number can be provided to the satisfaction of the RO.
