Electrical installations should be such that the safety of personnel and ship from electrical hazards will be assured. PSCO identified multiple splices that were installed improperly on the vessel's lighting system and generator sets. CCSSC Ch.2 31.1.3

Fixed fire detection and fire alarm systems required by this chapter should comply, as far as is practicable and reasonable, with the relevent requirements for these systems specified in the regulations for these systems specified in the regulations of the 1974 SOLAS convention. PSCO identified that the smoke detectors in the galley were dismantled and not capable of detection. CCSSC CH.3, 8.1

A Classification Society must not review, examine, survey or certify the construction, repair or alteration of a vessel in the United States unless it is approved under the provisions of this subpart. The vessel's Caribbean Cargo Ship Safety Certificate were issued by an unapproved Classification Society in Houston, TX, USA. 46CFR 2.45-10

The liferaft painter system shall provide a connection between the ship and the liferaft and shall be so arranged as to ensure that the liferaft when released and, in the case of an inflatable liferaft, inflated is not dragged under by the sinking ship. Vessels portside liferaft’s painter line was attached to the ship and not the weak link provided on the hydrostatic release. LSA 4.1.6.1 (CCSSC 4.3.1)

The satellite EPIRB and VHF EPIRB referred to in 5.13.1 and 5.13.2 should be: capable of floating free if the ship sinks and of being automatically activated when afloat. Vessels EPIRB’s housing was taped together rendering the hydrostatic release useless and unable to release if activated. CCSSC Ch. 5.13.3.3

Dead end wiring, exposed junction boxes/wires and alterations where wires were spliced and branching from existing power sources existed in excess throughout almost every space within the vessel. Most notable locations were the galley/messroom, crewmembers cabins, in the head/laundry room, and within the engine room. CCSSC 2.13.1.3

The owner or master of every ship to which this code applies should ensure that the condition of the ship and its equipment is maintained to conform with the provisions of this code to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship, persons on board or the environment. Vessels CCSSC requires the vessel to have on board 02 line throwing guns. Vessel currently only has 01 on board. CCSSC 1.11.1

Suitable arrangements should be made to the satisfaction of the Administration for embarkation into survival craft which should include means for illuminating the stowage position of survival craft and their launching appliances during preparation for and the process of launching, and also for illuminating the water into which the survival craft are launched until the process of launching is completed. The survival craft illumination on the starboard side of the vessel was not mounted and the wires were cut from their power source. The portside had to be tied to the railing of the ship as the mount had deteriorated and instead of illuminating the portside survival craft location, it instead illuminated directly away from the ship to port. CCSSC 4.10.1.2.2

The bulkheads and decks separating the machinery spaces of category A from control stations, corridors, accommodation spaces, stairways, services spaces, and cargo spaces should be so constructed to be capable of preventing the spread of fire to the unexposed side. The engine room door leading into the main passageway is unable to be closed, the door is not aligned, there is a bolt protruding out of the hinge, and the self-closing mechanism does not work appropriately. CCSSC Ch. 3.16.1.2

Stairways and ladders should be so arrange as to provide, from accommodation spaces, services spaces, control stations, machinery spaces and other spaces in which the crew is normally employed, ready means of escape to the open deck and thence to the survival craft. The access to a ladder in the engine room was blocked by a large pile of oily rans, a swab, and bucket of oily water. CCSSC Ch 3.16.4.1

All machinery and electrical installations, mechanical and electrical equipment and appliances, boilers and other pressure vessels, associated piping systems, fittings and electrical cables and wiring should be of a design and construction adequate for the service for which they are intended and should be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Vessel’s engine room had several missing deck plates, while others were unstable which would teeter and fall when stepped on. Multiple lightbulbs within the engine room were exposed and did not have covers protecting them. CCSSC 2.3

Steps and recesses in bulkheads should be watertight and as strong as the bulkhead at the place where each occurs. Vessel crew had drilled holes through multiple steel frames of porthole windows to run tv antenna lines into the vessel, no longer maintaining their watertight integrity. CCSSC 2.7.3.1

Where a cooking range requires an exhaust hood and duct, this should be fitted with a grease trap. The exhaust hood and duct were not fitted with a grease trap and had excessive grease in the duct. CCSSC Ch. 3.22.4

The fireman’s outfits or sets of personal equipment should be stored as to be easily accessible and ready for use. The access to fireman’s outfits were blocked by equipment and vessel stores. CCSSC Ch. 3.11.5

Handrails, grabrails, and handholds of sufficient size and strength should be provided where necessary in the opinion of the Administration for persons to hold on when the ship is severely rolling or pitching. The handrail and grabrail to the freezer from the galley are not secure. CCSSC Ch. 2.12.3

A fireman’s outfit should consist of personnel equipment comprising of an axe to the satisfaction of the administration. One fireman’s outfit did not have an axe located with the suit, as outlined in 3.2.7 of the vessel’s CCSSC Certificate. CCSSC Ch. 3.11.2.1.5

CCSSC 2.31.2 Electrical installations should be such that uniformity in the implementation and application of the provisions of this part will be ensured. PSCO observed an extensionn cord running from engine room to port side running light.

Electrical installations should be such that uniformity in the implemetation of the provisions of this part will be ensured (IEC publication 92). Replace twist on wire nuts with appropriate butt connectors on installed fire fighting device in engine room.

In every ship there should be provided for the protection of any space containing internal combustion type machinery having a total power output of 750 KW and above: one of the fixed fire extinguishing systems referred to in 3.10.1. The vessel's fixed CO2 System in the engine room is inoperable.

Every ship of less than 300gt should by 01 February, 1999, carry at least one satellite EPIRB capable of transmitting a distress alert. The vessel's EPIRB had an expired battery.

CCSSC Ch. 2 Part A 2.3 - All machinery, mechanical, and associate systems should be designed and constructed for adequate service for which they are intended and in working condition for the environment in which they operate. Vessel grounded and lost its starboard main engine.

The emergency fire pump should be capable of producing at least a jet of water of not less than 6M from one hydrant. Ships emergency fire pump was inoperable at the time of exam. 97 CCSS Code 3.3.7.2

The owner or master of every ship to which this Code applies should ensure that: after any survey of the ship under 1.10, no significant change should be made to other items. Ships required secondary line throwing appliance was expired. 97 CCSS Code 1.11.2

Rescue boat launching arrangements should be such that the rescue boat can be launched in the shortest possible time. No quick release hook installed on the rescue boat. 97 CCSS Code 4.10.2.1

All electrical installations should be so installed to reduce danger to persons on board. Ship had exposed wires on a 120v circuit in the engine room. 97 CCSS Code 2.3

Every ship of 100 gross tonnage shall carry a garbage management plan. Ship did not have a copy of the Garbage management plan on board. Marpol CONS 2011 Annex V Reg 10.2
