Each Captain of the Port may order a vessel to operate in the manner directed when the Captain of the Port has determined that such order is justified in the interest of safety by reason of weather, visibility, sea conditions, temporary port congestions, other temporary hazards circumstances, or the condition of the vessel. A-60 aft bulkhead in CO2 room found with open pipe penetration into adjacent hvac compartment. 74 SOLAS (14) II-2/14.2.1

Multiple grounding wires for lighting fixtures and ventilation fans found broken or missing throughout vessel exterior decks. 2. Reefer electrical gang boxes in cargo area found with broken covers sealed with electrical tape. 74 SOLAS (14) II-1/40.1.3

Internal combustion engines of 2,250 kW and above or having cylinders of more than 300 mm bore shall be provided with crankcase oil mist detectors or engine bearing temperature monitors or equivalent devices. Ten oil mist detectors found to be inoperable and not working as intended. 74 SOLAS (14) II-1/47.2

A fixed fire detection and fire alarm system shall be installed in machinery spaces. Marine inspector found broken fire detection head on the middle level of the engine room. 74 SOLAS14 II-2/4.1.1

Port lifeboat found with fire extinguisher that had not received annual servicing. 2. Starboard lifeboat found with multiple deteriorated/broken straps. 74 SOLAS (14) III/20.1

12107- Fire Protection Cargo Deck Area-Electrical Equipment shall not be fitted in enclosed cargo spaces unless essential for operational purposes...equipment shall be certified safe type for use in the dangerous environment to which it is exposed. Cargo hold #1 light found destroyed by previous event. Repair or replace light with fixture certified safe type for the enclosed cargo space. SOLAS 74 Amend(20 Consolidated) II-2/19.3.2

07199- Other (Fire Safety)- The ignitability of combustibles shall be restricted. #7 Cargo hold, centerline HFO line expansion joints were found missing flashing. A drip pan undeath the fitting had accumulation of oil. Vessel is required to replace the flashing and ensure fitting is secure to prevent leaks. SOLAS 74 Amend(20 Consolidated) II-2/4.1

07199- Other (Fire Safety) Means to control leaks of flammable liquids. #6 cargo hold had HFO line was found missing flashing at piping. Vessel is required to replace missing flashing. SOLAS 74 Amend(20 Consolidated) II-2/4.1

02199- Other(Structural Condition)- The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. Inspector identified corroded brackets on the exterior port quarter fire main risers. Vessel is required to repair/replace the bracket to orginal condition. SOLAS 74 Amend(20 Consolidated) I/11.a

02199- Other (Structural Condition)-The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. Make repairs to pin holed Grey Water overboard piping above turn in piping on inboard side of skin valve. SOLAS 74 Amend (20 Consolidated) I/11(a)

01104-Cargo Ship Safety Radio- The Coast Guard may delegate issuance of the certain international convention certificates to a recognized classification societies. A SOLAS Cargo Ship Safety Radio Certificate issued by Lloyd’s Register was found on board. Certificate issued on 10FEB2024 in Port of Houston, USA. Remove certificate and maintain valid FCC Safety Radio Certificate. 46CFR8.320(b)

01104-Cargo Ship Safey Radio- A certificate called a Cargo Ship Safety Radio Certificate shall be issued after an initial or renewal survey to a cargo ship which complies with the relevant requirements of chapter IV and any other relevant requirements of the present regulations. A valid Safety Radio Certificate issued by approved FCC licensed authority found with incorrect gross tonnage. Correct certificate upon return to US port. SOLAS 74 Amend (20 Consolidated) I/12.a.iv

Before the ship leaves ports and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Starboard lifeboat rudder is inoperable due to a cracked internal drive mechanism of the steering column. SOLAS 74 (2020 Con. Ed.) III/20.2

Immediately after addressing of resultant safety concerns, the owner, agent, master, operator, or person in charge, shall notify the nearest Sector Office, Marine Inspection Office, or Coast Guard Group Office whenever a vessel is involved in a marine casualty, consisting in an occurrence materially and adversely affecting the vessel's lifesaving equipment. Vessel master did not report marine casualty to the nearest Marine Inspection Office, USCG Activities Europe (ACTEUR). Marine casualty occurred in ACTEUR's marine inspection zone. 46 CFR 4.05-1(a)(4)

Before ship leaves port and at all times during the voyage, all life-saving appliance shall be in good working order. Marine Inspectors observed Port lifeboat could not be lowered in it's entirety to the water due to the remote brake cable not being adjusted correctly. Stop would hit entrance point of lifeboat. Crew had to adjust slack to remote brake cable in order for it to lower. Servicing company attended and corrected issue. Tech stated cable was too 100m, when the requirement is 49m.

Exhaust ducts from galley ranges shall meet the requirements of paragraphs 7.2.1.2.1 and 7.2.1.2.2: the ducts are insulated to "A-60" class standard throughout the accommodation spaces, service spaces or control stations. Marine Inspector discovered galley vent hood had hole in bottom section and temporarily repaired with a piece of metal sheeting.

Direct access to the embarkation deck by way of external open stairways and passageways shall be provided and shall have emergency lighting in accordance with regulation III/11.5 and slip-free surfaces underfoot. Marine Inspector discovered multiple emergency lights on the port and starboard side muster/embarkation stations extinguished.

Ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration. Marine Inspector discovered electrical junction boxes on aft superstructure missing protective caps.

Freeing ports, open rails and scuppers shall be fitted as necessary for rapidly clearing the weather deck of water under all weather conditions. Marine Inspector discovered clogged scuppers on upper deck aft of superstructure.

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II-1/42 or II-1/43, as appropriate. Marine Inspector discovered all flood lights on port and starboard side muster stations missing grounding wires.

Except as provided in regulations I/7(b)(ii), I/8 and I/9, while all reasonable steps shall be taken to maintain the equipment required by this chapter in efficient working order. Marine Inspector discovered aft anchor light falling off of mounting surface.

Guard rails or bulwark shall be fitted around all exposed decks. Guard rails on STBD Focsle had damage and broken weld. Guard rail on the FWD Port side of A deck had broken guard rails.

Rescue boats shall be stowed: in a state on continuous readiness for launching in not more than 5 min. Rescue boats outfit was stored in STBD life boat. Rescue boats steering column was wasted and loose. The breaker boat was broken and exposed to environment, and the rescue boat battery was stored in a milk crate.

Before ship leaves port and at all times during the voyage, all life-saving appliance shall be in good working order. Lifejackets had holes cut into them for previous outfit of lights. Lights were removed, leaving large 3” long by 2” deep hole. Rendering lifejackets not serviceable.

At least three two-way VHF radio telephones apparatus shall be provided for every passenger ship and cargo ship of 500 gross tonnage and upwards. VHR Radios on bridge were not stowed in accordance with Fire Control Plan. Two radios charging stations are hard wired in to locations that are not mentioned on Fire Control Plan or are not marked by the appropriate symbol.

Lifebuoys complying shall be stowed as to be capable of being rapidly cast loose and not permanently secured in any way and shall be fitted with self-igniting light. Self-igniting light of lifebuoy at bay 58 Port side did not work. Light was seized in bracket.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Fire hose on Port bridge wing had leak during testing of emergency fire pump.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Main salt water cooling seal failure resulting in salt water ingress.

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. Jacket water heater flange failure resulting in discharge of steam into engineering space.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Starboard bunker header pressure gauge inoperable.

The construction of doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated, this being determined in accordance with the Fire Test Procedures Code. Marine Inspector discovered galley fire door wasted at bottom edge and unable to automatically close completely.

Fire protection systems and appliances shall be maintained ready for use. Fire damper emergency generator room seized. Corrected on spot.

Main engine F/O injection pump control oil nonmetallic hoses were excessively worn, had no date of manufacturer. SMS requires verification of date of manufacturer for 10 year shelf-life and requires replacement if found worn. Accepted by LR during reflag.

Starboard lifeboat damaged upon operational lowering of lifeboat. Lifeboat kort nozzle & keel were damaged. The cable for the remote control wire parted causing the lifeboat to swing which caused the damage. Accepted by LR during reflag.

Port steering pump loss of phase alarm was found inoperable during operational test. Accepted by LR during reflag.
