Each towing vessel must be maintained and operated so as to minimize fire hazards. The aft starboard fuel tank contains a pin hole. Repair aft starboard fuel tank to the satisfaction of the attending USCG Marine Inspector.

Resolution: Satisfactory weld repairs and NDT completed for fuel tank pin-hole. Tank is fit for fuel service.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The Following alarms were not able to be tested: main engine low lubricating oil pressure, main engine high cooling water temperature, auxiliary generator engine low lubricating oil pressure, auxiliary generator engine high cooling water temperature, low hydraulic steering fluid levels and low fuel level for the day tanks. Test alarms in the presence of a USCG Marine Inspector.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Emergency lighting was inoperable throughout the engine room and accommodation spaces. Provide adequate emergency lighting for the engine room and accommodation spaces.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing in case of fire, each doorway, ventilator, and annular space around funnels and other opening into such spaces. Vessel did not have the means to cover the ventilation hoods of the engine and generator rooms. Acquire ventilation covers.
