In addition to the radiotelephone required by paragraph (b) of this section, each vessel described in paragraph (a) of this section while transiting any waters withing a vessel traffic service area, must have on board a radiotelephone capable of transmitting and receiving on the VTS designated frequency in Table 161.12 (c) (VTS) and VMRS centers, call signs/MMSI, designated frequencies, and monitoring areas). The VHF-FM radios did not have the MMSI numbers inputted into the radios, rectified on the spot.

Mechanical ventilation shutdowns are on a separate systems and are not set up for automatic shutdown operation.

The official number of the vessel, preceded by the abbreviation “NO.” must be marked in block-type Arabic numerals not less than three inches in height on some clearly visible interior structural part of the hull. The number must be permanently affixed to the vessel so that alteration, removal, or replacement would be obvious. If the official number is on a separate plate, the plate must be fastened in such a manner that its removal would normally cause some scarring of or damage to the surrounding hull area. Vessel did not have official number marked on the visible interior of the structure. Vessel is required to affix official number to the satisfaction of the attending marine inspector.

Each fuel shut off remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Vessel remote valves were not marked accordingly for purpose. Vessel is required to marked remote operation of valves satisfactory to the attending marine inspector.

Vessel was unable to provide towline records for maintenance and inspections.
