PSCO collected evidence that the vessel's SMS was not adequately being implemented throughout areas of the ship including: structural integrity, emergency lighting, weathertight & watertight integrity, and electrical installations. An external audit is recommended prior to departure.

The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. The arrangements shall ensure that the tightness can be maintained in any sea conditions. PSCO observed soft patch for steering gear room, a hatch above CO2 room, and hatches and vents leading into the Bosun Stores not maintaining weathertightness as evidenced by standing water and rust lines from water intrusion.

Resolution: PSCO received evidence from class and flag state that this was not a temporary repair to the vessel. Vessel uncovered the hatch and used standing water to verify that the hatch was watertight.
