Survival craft must be stowed in accordance with the requirements of § 199.130 of this chapter, as far as is practicable on existing towing vessels. Survival crafts hydrostatic release was expired. O/O is to replace hydrostatic release and install according to manufacturer's instructions prior to movement of vessel. 46 CFR 141.310

Except as provided in paragraph (a)(8) of this section, each towing vessel must have a fire-detection system installed to detect engine room fires. The owner or managing operator must ensure the following: The system is installed, tested, and maintained in accordance with the manufacturer's design manual. Fire detection panel's emergency source of power batteries were dead and in need of replacement. O/O to install new emergency source of power batteries for fire detection panel prior to movement. 46 CFR 142.330(a)(2)

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Vessel's foam fill plug on the stbd bridge wing life ring buoy was missing. O/O to repair IAW manufacturer's instructions or replace buoy. 46 CFR 141.230

Each towing vessel must be fitted with a general alarm that: Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station". In vessel's engine room and pump room, there were no posted verbiage regarding the general alarm. O/O to install signage for general alarm and ensure flashing light is red and not blue. 46 CFR 143.235(b)(3)

A navigation assessment entry must be recorded in the TVR, official log, or in accordance with the TSMS applicable to the vessel. The entry must include the date and time of the assessment, the name of the individual making the assessment, and the starting and ending points of the voyage or trip that the assessment covers. Vessel was not completed an official navigation assessment as defined in 46 CFR 140.635 and entering in TVR or log book. Nav assessment was also not getting re-evaluated throughout voyage and at end of watches. O/O to institute navigation assessments into management system and ensure proper logging. 46 CFR 140.635(d)

The design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society with equivalent experience, to comply with paragraphs (a)(1) through (6) of this section. O/O was unable to provide documentation that the fire detection panel was installed by professional engineer. O/O to provide documentation within 30 days. 46 CFR 142.330(a)(7)

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. Annual inspection must be completed prior to pushing, pulling or towing alongside operations.

Each towing vessel operating on oceans, coastwise, limited coastwise, or beyond 3 nautical miles from shore upon the Great Lakes must carry a Category 1, 406 MHz satellite Emergency Position Indicating Radio Beacon (EPIRB) that meets the requirements of 47 CFR part 80. EPIRB onboard had witness tab removed and EPIRB is considered activated. Provide and register new CAT 1 EPIRB or service EPIRB at authorized service station and provide report. 46 CFR 141.380

Vessel life raft was not rigged in a float free arrangement. Hydrostatic release was present and not rigged correctly. Corrected prior to end of inspection. 46 CFR 141.310/199.130

All lifesaving equipment must be tested and maintained in accordance with the minimum requirements of § 199.190 of this chapter, as applicable 46 CFR 141.235. 46 CFR 199.190 (b) Maintenance. (1) The manufacturer's instructions for onboard maintenance of lifesaving appliances must be on board the vessel. The following must be provided for each appliance (i) Checklists for use when carrying out the inspections required under paragraph (e) of this section. (ii) Maintenance and repair instructions. (iii) A schedule of periodic maintenance. (iv) A diagram of lubrication points with the recommended lubricants. (v) A list of replaceable parts. (vi) A list of sources of spare parts. (vii) A log for records of inspections and maintenance. Vessel does not have a maintenance plan for lifesaving equipment.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110, unless the towing vessel is limited to daytime operation, in which case no floating electric water light is required. 02 water lights onboard had expired batteries. Replacement batteries are to be the make and model specified on the light by the manufacturer. 46 CFR 141.360(c)(2).

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. 46 CFR 143.230 Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. Alarms were unable to be verified as no procedures were available on the vessel.

Reconnect the starboard grease hose in the #2 void.

Secure the oil leak at the port hydraulic ram in the #1 void.

Renew 03 check valves.

A towing vessel that is certificated for coastwise route must be equipped with either an approved 160-B semi-portable fire-extinguishing system or a fixed fire extinguishing system installed to protect the engine room. Vessels fixed fire system, including the flexible connections has not been serviced, and the vessel does not currently have a 160-B semi-portable. Provide fixed or semi-fixed system for engine room IAW regs prior to vessel departure. 46 CFR 142.315

Provide a line throwing device prior to operation on a coastwise route. Vessel restricted to limited coastwise route within 3 miles of shore. 46 CFR 141.385 & 141.370

Each towing vessel on a coastwise route must have a means to take blood pressure readings, splint broken bones, and supply large bandages for serious wounds. Vessel does not have a means to take blood pressure. Please provide, vessel restricted to limited coastwise route. 46 CFR 140.435

Provide garbage management plan per 46 CFR 151.51

Test required electronics at operating station in presence of a marine inspector. (to include: Radar, GPS, and AIS)

Update MMSI into VHF radios. 2 VHF radios have the incorrect MMSI, 1 does not have MMSI input. (Update official number into the FCC website, Station License has the NH state number).

Electrical hazard on starboard shaft drip pan junction box. Wires are exposed and need to be enclosed.

Port engine was being worked on, and was not able to be run during the initial certification. Start and run port main engine in presence of a marine inspector.
