The flammable product locker located on the starboard side was not provided with 40-B portable extinguisher located nearby. Install a 40-B portable extinguisher located near the flammable locker to the satisfaction of the attending MI. 46 CFR 142.225(d)

General alarm was found inoperable and unable to be activated at a operating station and sound throughout the ship.
