No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Drawings of alterations shall be approved before work is started unless deemed unnecessary by the Officer in Charge, Marine Inspection. Electrical panel P202 had circuits installed that are not on the approved electrical drawings. Circuit shall be tagged out and not used until plan review has been completed/approved.

Liquefied or non-liquefied gas is prohibited for cooking, heating, and lighting on ferry vessels, but may be used on other inspected vessels if the system in which it is used meets the applicable requirements of subpart 58.16 or subpart B of part 184 of this chapter, as appropriate, or is approved by the Commandant (CG-ENG). Multiple propane tanks had been found on board. All must be removed.

Electric installations and electric equipment must be inspected at the inspection for certification and periodic inspection to determine mechanical and electrical condition and performance. Generators over-speed, and low lube oil pressure alarm was not able to be tested. The engineer currently on board could not facilitate the testing for the generator tests/shutdowns. Provide tech report for these test.

Oxygen and acetylene cylinders located on main deck port side not stowed in a stowage rack as required by 46 CFR 147.1(b) & 147.60. 46 CFR 60(b) cylinders stowage and care of compressed gas cylinders must always be secured and, when not in use, must be stowed in a rack in an upright position, with the valve protection cap in place. Procure or fabricate a rack complying to the applicable CFR. To clear, statement with digital photo(s) to be emailed to the attending CG Marine Inspector.

Observed forward generator removed. Informed and records reflected the generators main stator windings were burnt, with generator shipped out for repairs. As required by 46 CFR 91.45-1 no repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. To clear, 3rd party generator repair shop report, statement from the chief engineer stating generator was reinstalled, tested, and operating as designed shall be provided to the attending CG Marine Inspector.

The plans required by paragraph (d)(1) of this section must be certified by a registered professional engineer to meet the design requirements in paragraph (d) of this section and in section VIII, division 1, of the ASME Boiler and Pressure Vessel Code. The pressure vessel on board the barge has a valid U1A with the ASME "U" symbol however it does not meet the additional requirements found in 46 CFR 54.01-5(c)(3) for pressure vessel use on board inspected vessels. After verification from a P.E. or the Marine Safety Center, a Coast Guard Symbol shall be applied as required by 46 CFR 54.10-3(c).
