PSCO discovered vessels secondary means of allowing full flow relief of vapor by way of P/V Breaker is not fitted with a device to prevent the passage of flame.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirement which may be established by the Company. No SMS procedures were provided to check self contained air support system on stern launch lifeboat or to check the foam level in the tank for deck foam, both of which were below the required level as noted in flag state inspection.
An internal audit is required within 90 days.
COC not issued due to the above deficiencies, re-examination for COC to be completed after deficiencies 1-3 have been corrected.
