A hatch exposed to the weather must be watertight. The gasket material for the forward and aft hatches did not provide a watertight seal. Renew the hatches to prevent the ingress of water. 46 CFR 179.360

Boundaries that separate machinery spaces from accommodation spaces, service spaces, and control spaces must be lined with noncombustible panels or insulation approved in accordance with § 164.009 in subchapter Q of this chapter, or other standard specified by the Commandant. The vessel was constructed with general purpose resin. The under deck of the engine room compartment did not have noncombustible panels or materials. Install fire retardant insulation or similar material. Prior to the installation, provide the specs of the materials to be used to the USCG for approval. 46 CFR 177.410

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. Fuel was leaking from the starboard and port side fuel shut off valve hoses. Repair the leaks. 46 CFR 182.455

Electrical equipment must be installed to protect from electrical hazards. Several dead end wires were found underneath the operating station. Properly remove or cover the wires with a junction box. 46 CFR 183.200

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel, such as electrical wiring and propulsion and power supply systems, must not be made without the approval of the OCMI, except during an emergency. Proposed alterations must be approved by the OCMI before work is started. MI observed that in the engine room, the generator had been replaced by an inverter, prior to receiving approval by [name withheld] approval in this project is necessary and 110v outlets for inflatable slide shall not be used until proper approval. Also provide information and work conducted by contractor in writing to the CG. 46 CFR 176.700(a) and (b) 50 C

Code 08199: On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette; (2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and (3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. The high bilge water aduo alarm (ring bell) underneath the operating station was found inoperable. 46 CFR 182.530 Code 17C.

Code 07110: A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. All three fire buckets are 5 gallon, instead of the required 2.5 gallons. 46CFR181.610 16C.

Provide an audible alarm at the operating station to indicate a high water level in the engine compartment (visual alarm is operable).

Install batteries in each life float's floating waterlight.

Provide a valid Certificate of Documentation prior to carrying passengers.

Provide proof of Employee Assistance Program training for all crewmembers and supervisory personnel working in a safety sensitive position. Supervisory personnel must receive at least 60 minutes of training.

During an initial inspection for certification the owner or managing operator shall conduct all tests and make the vessel available for all applicable inspections discussed in this paragraph, and in Subpart H of this part, to the satisfaction of the cognizant [name withheld] cognizant OCMI to conduct new to zone inspection prior to the carriage of passengers.
