The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Cracked welds were identified by Sector Charleston between frames 1 and 33. All cracks that are not adjacent to waterside hull shall be repaired and monitored as outlined in repair proposal to the satisfaction of the attending MI.

MI's observed cracked welds in the forward void spaces on the inboard side of both hulls. Longitudinal were bowed on both sides of the vessel. O/o to provide repair proposal and complete temporary repairs to the satisfaction of the local OCMI.

46 CFR 121.610 (b) On a vessel of more than 65 ft in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members [name withheld] system is not audible in the following spaces with the doors closed: main forward lounge (category 7), main deck common head (category 6), crew lounge (category 6), and ship’s office (category 6). Additionally, the emergency over-ride functionality is currently not working properly, making the PA system inaudible in any space where the volume selector switch has been turned down. Provide speakers for sufficient PA coverage in all accommodation spaces, and prove proper operation of over-ride functionality.

46 CFR 118.400 (f) 46 CFR 76.27-15 (d) Except for continuously manned operating stations as allowed by paragraph (f) of this section, each accommodation space, control space, and service space must be fitted with the following systems: (1) a smoke actuated fire detection system of a type approved by the Commandant that is installed in accordance with 46 CFR part 76. Fire detectors fitted in passenger cabins must also emit, or cause to be emitted, an audible alarm within the cabin when activated. The smoke detectors that form part of the type approved interconnected fire detection system do not alarm in the passenger cabin when activated. Per the fire control plan, independent battery operated smoke detectors are to be fitted in each stateroom, but not all were installed at completion of inspection.

46 CFR 120.432 (c) 46 CFR 120.434 46 CFR 112.15-1 (c), (d), and (f) A vessel of more than 65 ft with overnight accommodations for more than 49 passengers must have an emergency lighting system that complies with Part 112 in subchapter J of this part and must have floodlights for illuminating the stowage position and embarkation station of lifeboats, liferafts, rescue boats, and rescue platforms, where installed. The following emergency lighting must be arranged: lighting for passageways, stairways, and escape trunks in passenger quarters, crew quarters, public spaces, machinery spaces, and work spaces sufficient to allow passengers and crew to find their way to pen decks and to survival craft, muster stations, and embarkation stations with all fire doors closed; illuminated signs with the word “EXIT” in red letters throughout a passenger vessel so that the direction of escape to the open deck is obvious from any portion of the vessel usually accessible to the passengers or crew; at least one light in each space where a person may be maintaining, repairing, or operating equipment, or transiting, such as public spaces, work spaces, machinery spaces, workshops, galleys, windlass rooms… Multiple emergency lights listed on drawing no. CGS-H145 SC-ELP1LS SHT1 , Rev #01, found to be inoperable or missing at time of inspection. Provide sufficient emergency lighting to cover all above-mentioned areas.

46 CFR 116.415 A-Class bulkheads or decks must be so insulated with approved structural insulation. Penetrations in A-class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent he passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. The following structural fire protection arrangements were not able to be verified as fully compliant with the applicable requirements by the completion of inspection: -Cable penetrations in the overhead of the captain’s cabin, underneath the IBAs. -Bulkhead between aft main passageway and aft sky lounge, 12” down from ceiling. -Cable penetrations in bulkhead between forward main passageway and forward main lounge. -Pipe penetration above entrance door to galley stores. -Several spots of missing insulation in bulkheads of ship’s office. -Pipe penetration on port side of z-drive room. -Duct penetration above entrance door to crew galley.

Various electrical installations were still in progress at completion of inspection. Electrical installation requirements, including required supporting without use of plastic wire ties, no dead end wires, etc. to be verified.

The shipboard tests were satisfactorily completed in the presence of the attending Marine Inspector; re-submit the the updated QFAs/DVTPs to MSC for final review and approval.
