Each liferaft of group of liferafts shall be stowed with a float-free arrangement complying with the requirements of paragraph 4.1.6 of the code so that each floats free and, if inflatable, inflates automatically when the ship sinks. PSCO discovered the painters line for the liferaft was not attached to week link on the vessel, which would prevent free float/automatic inflation of the liferaft.

All electrical apparatus shall be so constructed and so installed as not to cuase injury when handled or touched in the normal manner. PSCO discovered a bare electrical wire as the outer conduit had been damaged/removed on the #3 L/O purifier.

PSCO discovered an unapproved patch on a ballast water crossover pipe in vicinity of the ballast pumps.

The machinery, boilers and other pressure vessels, associated piping systems and fitting shall be of design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaced and other hazards. PSCO discovered an exhaust leak on the piping aft side of the exhaust trunk of the main diesel engine.

Means shall be provided to control leaks of flammable liquids. PSCO discovered an unapproved patch/repair on the #3 fuel purifier. The patch/repair had been applied to the expansion joint/pipe.
