The company should establish procedures, plans, and instructions including checklists, for key shipboard operations. Following a pollution incident on 25Jan2023, PSC officer attended vessel and noted the appropriate procedure/checklist was not adhered to by shipboard personnel while transferring sludge between tanks. The company is conducting an internal audit. The USCG also requests an external audit be conducted focused on the pollution incident. 74 SOLAS (20) IX/3.1 ISM Code 7

The master and crew failed to identify the cause of an oil spillage during an internal transfer procedure in a timely manner upon the request of the Coast Guard, which questions the overall competency of the crew in conducting key shipboard operations involving oily substance. 74 SOLAS(20) IX/3.1 ISM Code 9.2

Oil sludge tanks shall not be arranged with any piping that has direct connection overboard, other than the standard discharge connection. Oily waste from the sludge tank was released overboard through a pipe other than the standard discharge connection and the crew was unable to determine how this release occurred. MARPOL I/12.3.4
