AN AUDIBLE AND VISIBLE ALARM SYSTEM TO GIVE A WARNING ON BOTH SIDES OF THE AIRLOCK SHALL BE PROVIDED. THE VISIBLE ALARM SHALL SOUND IF DOORS ON BOTH SIDES OF THE AIR LOCK ARE MOVED FROM THE CLOSED POSITION. PSCO OBSERVED THE AUDIBLE AND VISUAL ALARM OUTSIDE THE MOTOR ROOM AIRLOCK NOT OPERATION AND THE LIGHT IN THE MOTOR ROOM NOT OPERATIONAL WHEN BOTH DOORS TO THE AIRLOCK WERE OPEN.
