Fuel Shut Off Requirements. To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shut of valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime move. The valve must be installed in the fuel piping directly outside of the fuel oil supply tank. The valve must be operable from a safe place outside of the space where the valve is installed. The remote shut off valve was not located at the tank, rather installed at a secondary valve approximately two feet away from the valve installed at the tank. Vessel shall install a remote valve to the valve directly outside of the tank for all fuel lines supplying fuel to the propulsion engines and generator prime mover. Note that piping was not marked or labeled to identify if additional piping requires remote shut offs. (see next deficiency). 46 CFR 143.260(b)-(d)

Piping for flammable or combustible liquid, seawater cooling, or fire fighting systems must be fitted with isolation valves that are clearly marked by labeling or color coding that enable the crew to identify its function. No fuel line valves were labeled or color coded. Vessel shall properly mark all fuel line isolation valves and identify any additional valves which may need to be marked. 46 CFR 143.250(c)

No person may operate any vessel equipped with installed toilet facilities unless it is equipped with an operable type II or type III device. The vessel's type II MSD overboard discharge piping was leaking/spraying sewage excessively into the engine room when in operation causing a safety hazard. Vessel shall repair all leaks to MSD piping system. 33 CFR 159.7(a)

An exposed hazard such as gears and rotating machinery must be protected by a cover, guard, or rail. The starboard generator was missing its guard on the side ventilation opening, exposing the rotating gear and causing a potential safety hazard to crew members on board. Vessel shall install cover or guard over opening. 46 CFR 144.820
