A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. MI observed inoperable ring buoy waterlight due to dead battery. Restrict to daytime operation until waterlight is operational.

A life float must be approved under approval series 160.027 or other standard specified by the Commandant. Each life float must meet the requirements in subpart 160.010 of this chapter. Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. The material must be in strips at least 50 mm (2 in.) wide extending from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. MI observed deteriorated retroreflective material on the life floats. Replace retroreflective material.

A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons to be transported. Deficiencies 02-05, 7-17, 19-21, 23, 24, 26, and 27 provide objective evidence that the vessel is substandard and not proactively managing technical and operational requirements. Vessel must complete a satisfactory COI re-inspection prior to any movement.

At each inspection the firefighting equipment of all vessels shall be inspected. The marine inspector shall conduct the following examinations: All hand portable fire extinguishers shall be examined for excessive corrosion and general condition. Vessel’s portable fire extinguisher for the upper passenger accommodation space was discharged with the indicator in the red and required servicing. 46 CFR 176.25-25(a)(OLD CFRs)

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. The last annual servicing of fixed and portable extinguishers exceeds one year.

Provisions shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected. The starboard E/R vent closures failed to close due to excessive corrosion. 46 CFR 181.20-35(a) (OLD CFRs)

A vessel not required to have a power-driven fire pump by 181.300 must have at least three 2.5 gallon bucketswith an attached lanyard, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel only had two fire buckets onboard.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. An excessive accumulation of clutter and debris created a fire hazard and obstructed emergency exits in the forward and aft accommodation space.

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. The waterlight is inoperable. Vessel is restricted to daytime operations only until rectified by attending marine inspector.

A vessel must have one or more ring life buoys. The ring life buoy is unserviceable and must be replaced.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry—(1) Six hand red flare distress signals approved in accordance with § 160.021 … and (2) Six hand orange smoke distress signals approved in accordance with § 160.037. All hand red flares and hand orange smoke distress signals on the vessel are expired.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments, under all practical conditions. The lazarette submersible bilge pump wiring is disconnected rendering the pump inoperable. The port engine room submersible bilge pump is not mounted. 46 CFR 182.25-1(a)(OLD CFRs)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and the vessel from electrical hazards, including fire and electrical shock. The vessel had numerous wiring issues which posed a major hazard to passengers and crew. The hazards identified include: deteriorated battery cable connection in port E/R, dead end wiring on starboard E/R battery installation, dead end wiring at operating station adjacent to VHF radio, and various exposed and dead end wiring throughout vessel.

The intent of this part is to ensure that the scantlings, material, fastenings, workmanship, and arrangement of each vessel are as such to provide for safe operation in accordance with the terms of its COI. Primary consideration shall be given to the provisions of a seaworthy hull. Vessel has excessive water intrusion coming from both propeller shafts, causing the automatic bilge pumps to frequently operate.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high-water level in each of the following normally unmanned spaces: A machinery space bilge, shaft alley bilge, or other spaces subject to flooding from sea water piping. Vessel’s bilge alarm in the forward void does not provide a visual indication at the operating station and vessel does not have bilge alarms for the engine room or lazarette.

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines. The shutoff valve at the tank shall be operable from outside the tank compartment. The emergency fuel shutoff valve is inoperable as designed and required vice grips to operate. 46 CFR 182.20-40(b)(3)(OLD CFRs)

The equipment required for life floats must meet the following specifications: Each paddle must be of at least 4 feet in length, lashed to the life float to which it belongs, and buoyant. Vessel’s paddles were not secured to their respective life floats and were not labeled with the vessel’s name.

Firefighting, abandon ship, and man overboard drills and training shall be logged or otherwise documented for review by the coast guard upon request. The drill entry shall include the following information: date of the drill and training and a general description of the drill scenario and training topics. Vessel did not have a logbook and was not recording any required drills and training.

Each vessel must be equipped with at least two operable portable battery lights. One of the portable lights was inoperable with dead batteries.

Complete and simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and the CO2 cylinder storage room. The instructions for local CO2 control were missing. 46 CFR 181.20-15(d)(OLD CFRs)

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: “Emergency Exit, Keep Clear”. The emergency escape markings were worn and illegible. 46 CFR 177.30-15(a) (OLD CFRs)

An individual may not be engaged or employed, including self-employment, on a vessel in a position as master, operator, or person in charge for which a credential is required by law or regulation unless all crewmembers covered by this section are subject to the random testing requirements of this section. The master employed onboard the vessel during the inspection was not enrolled in the vessel’s DAPI program.

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of sub chapter A of this chapter shall comply with the requirements of § 4.06 in sub chapter A of this chapter. Vessel's required alcohol testing swabs were expired.

Rails shall consist of evenly spaced courses and the spacing between courses shall be not greater than 12 inches. On passenger decks of vessels’ engaged in excursion type operation, rails shall be at least 42 inches high. The space below the rail shall be fitted with bulwarks, chain link fencing, or the equivalent. The space between rail courses were approximately 20 inches, exceeding the rail course requirements for an excursion vessel. 46 CFR 177.35-1(b)&(c)(OLD CFRs)

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and conduct the following inspections of the hull structure: Inspect all railings and bulwarks and their attachment to the hull structure. The starboard railing, adjacent to the operating station, is unsecured due to the use of unsuitable wood screws for the hinge pin connection. 46 CFR 176.25-5(a)(4)(OLD CFRs)

Life floats must not be stowed in tiers more than 4 feet high. When stowed in tiers, the separate units must be kept apart by spacers. Spacers between lifefloats were missing.

Lighting fixtures shall be contrasted in accordance with the requirements of UL Inc. Standard for Marine Type Electric Lighting Fixtures. An overhead light in the E/ R was missing its globe. 46 CFR 183.10-20(l)(OLD CFRs)

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. The waterlight lanyard is deteriorated and frayed. Vessel is restricted to daytime operations only untilrectified by attending marine inspector.

If a vessel operates on exposed or partially protected waters, an opening port light is not permitted below the weather deck unless it has an inside, hinged dead cover. 46 CFR 171.119(a)(OLD CFRs)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel lost operation of the electronic throttles resulting in the inability to control propulsion.

Vessel must be equipped with LF of an aggregate capacity that will accommodate 100% of total number of persons on a limited coastwise route in cold water. Vessel has (1) 22 person LF and will be restricted to 22 total persons during cold water months on a limited coastwise route.

No marine employer shall hire any person as crewmember without a passed chemical test. Licensed mariner needs pre-employment screening.

Marine employers must ensure crewmembers meet chemical testing regulations. Licensed mariner was not in consortium.

Vessel neds FCC Station license for maritime service.

Vessel needs a detailed inspection by FCC and obtain a Communications Act Radiotelephony Certificate.

Must have on board FCC type accepted CAT 1 406 MHz EPIRB. MI found no EPIRB registration.

Marine Employer shall comply with requirements of 4.06 in subchapter A. MI found no DAPI Consortium enrollment letter.

Vessel must be provided with satisfactory arrangement for draining any water tight compartment. MI found bilge system inoperable.

Double hose clamping is required. MI found multiple area without double clamps on bilge syste.

Vessel must submit plans for installation of monitoring devices to ensure the wakefulness of watchmen. No plans submitted for watch alert system.

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. If webbing is used to secure life lines and pendants, it must be at least 50 mm (2 in.) wide and must have a breaking strength of at least 3.4 kN (750 lb.) for apparatus of under 25 persons capacity, and 6.7 kN (1,500 lb.) for apparatus of 25 persons capacity and higher. 22 person life float was found with two securing points for the life line severed. Repair or replace life float. Until a serviceable life float is provided, vessel is restricted to 1 NM from shore while carrying passengers or cargo.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and all other hazardous situations must be corrected. At required hull exam vessel was identified that both port and stbd propellers were not in serviceable condition. Both propellers had severe cracking and damage/broken trailing edges. Vessel to make shaft/props, and all associated components available for inspection during next hull exam and correct as required.

The owner or managing operator shall make a vessel available for dry dock exam and internal structure exam, as required. Vessel did not complete a satisfactory hull exam. Vessel is required to make vessel available for hull exam and credit dry dock. All items to be completed to the satisfaction of the attending marine inspector and prior to carrying passengers for hire.

Footholds, handholds, ladders, and similar means provided to aid escape, must be suitable for use in emergency conditions, of rigid construction, and permanently fixed in position, unless they can be folded, yet brought into immediate service in an emergency. No ladder installed for aft accommodation space egress hatch.

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. BMDE’s and SSDG does not secure up CO2 Activation

Each survival craft must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. 01 Life Float was found deteriorated and outer coating cracked.

Machinery and fuel tank spaces shall be seperated from accommodation spaces by vapor tight bulkheads. MI discovered bulkhead penetrations in generator room to not be vapor tight. Vessel needs to seal penetrations.

A visual and audible alarm must be provided at the operating station to indicate a high water level. The vessel failed to produce 03 of 04 bilge high level alarms at the operating station.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Due to the electrical problems onboard the vessel: the vessel has failed it’s annual inspection. Vessel must complete an annual inspection prior to movement.

Adequate emergency lighting shall be fitted along the line of escape. Vessel not equipped with emergency lighting in forward accommodation space. Vessel to provide space with adequate emergency lighting.

Each survival craft must be secired to the vessel by a painter with a float free link. MI discovered vessel did not have a painter or float free link.

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. The vessel has not completed a satisfactory annual inspection. Vessel cannot carry passengers until a satisfactory annual inspection is completed.

At each initial and subsequent inspection the owner or managing operator shall be prepared to test the steering system of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Port and starboard rudder packing leaking excessive water into the bilge. Provide means to repack rudders to stop the ingress of water. Rectify deficiency prior to movement.

The marine inspector may require any part or all of the propeller shafting to be drawn for examination of the shafting and stern bearing of a vessel whenever the condition of the shafting and bearings are in question. Port and starboard main shaft cutlass bearings worn passed 50%. Port shaft cutlass bearing has slipped out of the strut and is close to contacting the propeller. Pull port and starboard shafts for inspection and replace port and starboard main strut cutlass bearings prior to carrying passengers.

A vessel is inspected for compliance with the sandards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the OCMI as good marine practice. Battery Charger in generator space has its power cord running up the foldable stairs to the next deck and is plugged into an outlet. The cord is being pinched and chaffed by the stairs. Provide a plan to hardwire the battery charger into to the vessels circuit panel with proper overcurrent protection.

FOR FIXED GAS FIRE EXTINGUISHING SYSTEM: DEVICE MUST BE PROVIDED TO SHUT DOWN POWER VENTILATION SERVICING PROTECTED SPACE. SPRING ON PORT ENGINE ROOM VENTILATION CLOSURE WASTED. REPLACE AND PROVE PROPER OPERATION.

LIFE FLOAT MUST HAVE WATERLIGHT. WATER LIGHT NOT FUNCTIONAL. PROVE PROPER OPERATION.

EXPOSED ELECTRICAL WIRINGS, JUNCTIONS, AND IMPROPERLY SECURED WIRES IN ENGINE AND GENERATOR ROOMS. RECTIFY DEFICIENCIES TO SATISFACTION OF ATTENDING COAST GUARD INSPECTOR.

MASTER SHALL ENSURE VESSEL COMPLIES WITH ALL APPLICABLE STABILITY REQUIREMENTS. VESSEL HAD INCORRECT LABELING AT LADDERWELL FOR MAX PASSENGER COUNT ON UPPER DECK. PROVE PROPER LABELLING.

Valve did not close properly upon testing.

Provide a valid FCC ship radio station authorization certificate.

Provide updated charts for navigation.

Provide a vald first aid kit or required contents.

Renew retroreflective tape for 06 lifejackets on bow.

Renew retroreflective tape for stern ring buoy.

Insert horizontal rail courses in bow railing so no verticle space exceeds 12 inches. To also include PORT/STBD embarkation zones. Passengers restricted from accessing bow. To be completed to the satisfaction of attending CG Marine Inspector.

Provide new retroreflective tape for all lifejackets as identified. To be completed to the satisfaction of attending CG Marine Inspector.

Provide up to date charts for Catalina Island and the LA/LB harbors. To be completed to the satisfaction of attending CG Marine Inspector.
